What are the best treatments for perianal abscesses?

The treatment of perianal abscess requires active response, because the longer you delay, the greater the impact and harm it will cause. (I) Treatment of simple abscess

In the lithotomy position or side-lying position, using local anesthesia or spinal anesthesia, make radial incisions at the abscess site. After the pus is released, insert the index finger to explore the size of the abscess cavity and separate its intervals. If necessary, cut the skin at the edge of the incision a little to facilitate drainage, and finally insert a Vaseline gauze strip into the abscess cavity for drainage.

(ii) Abscess with abscess cavity communicating with anal fistula

After opening the abscess, use a probe to carefully check the internal opening, then open the fistula, appropriately remove the skin and subcutaneous tissue, and slightly remove the tissue around the internal opening to ensure smooth drainage. If the internal opening is deep and the fistula passes through the anal sphincter, thread hanging therapy can be used. The advantage of the above surgery is that the abscess is cured in the first stage and no anal fistula will be formed. However, in acute inflammation, when it is difficult to find the internal opening, you should not search blindly to prevent the spread of inflammation or the formation of a false channel. Only make an incision and drain the pus, and then perform anal fistula surgery after anal fistula is formed. The advantage of the second-stage surgery is accurate effect and high cure rate.

3. TCM treatment of perianal abscess

1. Anal abscess due to excess syndrome: The syndrome is caused by internal accumulation of dampness and heat and endogenous toxicity. Symptoms include obvious redness, swelling, heat and pain in the anus, constipation, short and dark urine, red tongue with yellow and greasy coating, and rapid pulse. Treatment method: clear away heat, detoxify and remove dampness.

2. Anal abscess due to deficiency syndrome: The syndrome is caused by yin deficiency, internal heat and endogenous dampness and heat. Symptoms include unchanged anal skin color, unclear boundaries, no bulge or slight bulge, thin white pus, general fatigue, false heat, red tongue with greasy coating, and thin pulse. Treatment method: Nourish yin, clear heat and remove dampness.
