Gastrointestinal symptoms may include nausea, vomiting, upper abdominal distension and pain, and in severe cases, hiccups.

Arrhythmias often occur within 1 to 2 weeks of onset, and are most common within 24 hours. Ventricular arrhythmias are the most common, especially premature ventricular contractions. Atrioventricular and bundle branch blocks are also common.

Hypotension and shock Shock usually occurs within a few hours to a week after onset and is mostly cardiogenic.

Heart failure is mainly acute left heart failure. It is caused by weakened myocardial contractility or uncoordinated contraction after infarction.

3. Physical signs

The heart borders are enlarged, the heart rate is fast, the first heart sound at the apex is weakened, the fourth heart sound and gallop rhythm may appear, and there is usually a pericardial friction sound after 2 to 3 days. A rough systolic murmur or a click in the middle and late stages of systole may occur in the apical area, which is caused by dysfunction or rupture of the mitral papillary muscles, and may lead to various arrhythmias.
