Postoperative care for bladder cancer is very important

Bladder cancer is easy to be ignored in the early stage of the disease because the symptoms are not very clear. By the time you find yourself sick, the condition is often very serious and requires surgery. At this time, we must not be careless and must be careful to do a good job of post-operative care.

1. Be careful to prevent anastomotic fistula after surgery

In both ileal cystectomy and partial cystectomy, there is a possibility of fistula at the anastomosis. In other words, the incision edges of the joints do not heal well, and urine leaks into the abdominal cavity. This is one of the more common postoperative complications. Once it occurs, it takes a long time to heal. In some severe cases, reoperation is required.

2. Detect fistulas early

For patients who have undergone partial cystectomy, the amount of flushing input and output should be observed. If the amount of fluid discharged is less than, equal to, or more than 400 ml of the flushing input, the occurrence of fistula should be highly suspected.

The purpose of the abdominal drainage tube is to drain the intra-abdominal fluid. The amount drained will gradually decrease after surgery. If the drainage volume suddenly increases and the urine appears light yellow, fistula is likely to occur.

Patients with fistulas often have fever.

3. Prevention and treatment of fistula

If the drainage is blocked, the amount of fluid in the bladder is large, and the pressure and volume exceed a certain limit, it will affect the healing of the anastomosis. Once a fistula occurs, the doctor will decide whether to perform conservative treatment or surgery after careful examination. Conservative treatment is to promote healing by taking measures such as strengthening nutrition. However, some people still need surgery again in the end.

Improving nutrition and maintaining patency of the bladder drainage tube are effective means to prevent fistula after surgery. Malnutrition should be corrected before surgery. After the surgery, while strengthening dietary nutrition, intravenous nutrition (such as albumin infusion) is sometimes required.
