Sarcoidosis is a chronic disease that can invade multiple systems throughout the body, 90% of which involve the lungs. Its basic lesion is the formation of non-caseous necrotic granulomas. The disease is more common in middle-aged and young women, with the lungs and hilar lymph nodes most commonly affected, but can also affect superficial lymph nodes, skin, eyes, tonsils, liver, spleen, bone marrow, etc.

2. Pulmonary nodules

There are many causes and types of diseases that cause diffuse nodular shadows in the lungs. Some diseases are limited to the lungs, while others are lung manifestations of systemic diseases.

Miliary nodules are divided into:

(1) Interstitial nodules refer to nodules located around the parahilar bronchovascular bundles, in the axial interstitium of the lobules, and in the surrounding interstitium (such as under the pleura, beside the interlobar fissures, and on the interlobular septa). They usually have clear edges, but the vascular texture is obscured and blurred. The most common ones are lymphangitis and carcinomatous lymphangitis.

(2) Air cavity nodules are nodules with fuzzy edges and uniform density, ranging in diameter from a few millimeters to 1 cm. They occur around the terminal and respiratory bronchioles, are located in the lobules, and have clear vascular textures. It is common in pneumoconiosis or extrinsic allergic alveolitis, etc.

(3) Random nodules: Nodules are irregularly or randomly distributed, and are most common in miliary tuberculosis and hematogenous metastases.
