What's the matter with 10 in urine routine occult blood

Many people don't pay much attention to what occult blood in urine routine test is 10, but we all know that if it is caused by some urinary tract infection, then it will cause great harm to reproductive health, so we must have a scientific understanding of its cause. If it is only 10 and there are no other adverse symptoms, it should be closely checked.

1. A positive urine occult blood test paper reflects that there are more red blood cells in the urine with hemoglobin and myoglobin, which is called hematuria. Normal human urine contains no red blood cells, or occasionally contains trace amounts of red blood cells (0 to 2 per high-power field of view). After strenuous exercise, heavy physical labor or standing for a long time, transient trace amounts of red blood cells may appear in the urine. This is normal and has no clinical significance. However, if a large number of red blood cells are frequently found in the urine, it indicates an abnormality and should be closely examined.

Second, the diagnostic criteria for hematuria are :

① Fresh morning urine without sediment can be directly used for smear examination, with >1 red blood cell in every 2 to 3 high-power fields.

② Microscopic examination of urine sediment: >3 red blood cells per high-power field; or red blood cell count using a cowbag Fahrenheit counter >8000/ml, or urine red blood cell excretion rate >100,000/hour.

③12-hour urine sediment red blood cell count (Addis count)>500,000. If the urine specific gravity is too low, the urine is alkaline, or the urine sample is left for too long, the red blood cells in the urine will be destroyed. Even though there is hematuria, no red blood cells may be detected in the urine sediment. The most common causes of hematuria are urinary system diseases, among which glomerular diseases, stones, infections and tumors are more common. The presence of free hemoglobin in the urine is called hemoglobinuria. Normal plasma contains a small amount of free hemoglobin bound to haptoglobin. Due to its large molecular weight, it cannot be filtered through the normal glomerular filtration membrane, so hemoglobin does not appear in the urine of normal people. During acute hemolysis, a large number of red blood cells are destroyed, and the destroyed red blood cells release free hemoglobin. When the free hemoglobin exceeds the binding capacity of haptoglobin, the unbound free hemoglobin is excreted from the kidneys, resulting in hemoglobinuria. Occult blood was found in the urine, but no red blood cells were found on microscopic examination.

3. Clinical significance of urine occult blood test

It is mainly used to identify hemoglobinuria, and its positive test does not mean whether or not one has nephritis. Occult blood in urine may be positive in cases of blood transfusion reactions due to blood incompatibility, favism, paroxysmal hemoglobinuria, malaria, extensive burns, and inflammation caused by certain drugs or poisons. Hemoglobinuria may occur during attacks of various hemolytic anemias. In addition, patients with paroxysmal nocturnal hemoglobinuria and allergic hemoglobinuria may test positive during the attack period.
