If it is apical periodontitis, root canal treatment is recommended to completely drain the inflammatory secretions in the apical area and finally fill the tooth.

Symptoms of apical periodontitis

1. Acute apical periodontitis

In the early stage, the affected tooth will have mild pain. At this time, the pain can be temporarily relieved by clenching the affected tooth. As the inflammation worsens, the patient's teeth will elongate and light tapping of the affected tooth will cause pain. In apical periodontitis, the pain is spontaneous, persistent, and localized, and the patient can clearly point out the affected tooth. If the acute apical periodontitis continues to develop and forms an acute apical abscess, the pain will intensify, the percussion pain will be obvious, and there will be persistent throbbing pain. The pus spreads to the subperiosteum, and the pain and swelling are obvious. Once the pus penetrates the periosteum and reaches the submucosal area, the pain is relieved than before.

2. Chronic apical periodontitis

(1) Apical granuloma

Generally there is no spontaneous pain, only discomfort in chewing, weakness in biting, a strange sensation when percussed, and a feeling of elongation of the affected tooth. At this time, the pulp is mostly necrotic, and an acute attack may occur when the body's resistance is low.

(2) Apical abscess

There are usually no subjective symptoms. There may be fistulas in the mucosa of the apical area of ​​the affected tooth. Granulation tissue often proliferates at the fistula opening, and pus may be discharged from the fistula. Due to the drainage of the fistula, it is not easy to turn into acute inflammation.

(3) Apical cyst

There are usually no subjective symptoms, the teeth become discolored, and dental films can show cysts at the root apex. If the cyst increases, it forces the surrounding bone to be absorbed, and the mucosa at the root apex of the affected tooth will appear as a semicircular bulge, feeling like a ping-pong ball.

1. Open pulp drainage

In the acute phase, the pulp cavity should be opened and the root pulp removed to ensure that the root canal is unobstructed and that inflammatory substances can be drained from the root canal. After the pulp is opened, a soft cotton wick can be placed at the root canal opening to facilitate drainage.

2. Incision and drainage

During the acute suppurative stage, pus reaches the subperiosteum or submucosal membrane. An incision is made at the obvious location of the abscess in the corresponding apical area of ​​the affected tooth to drain the pus.

3. Root canal therapy

After the acute phase is relieved, the infection in the root canal is completely removed, the root canal is tightly filled, and permanent treatment is performed.

4. Apical scaling

This surgery is performed to eliminate lesions in the apical area when the lesions are large, but it is limited to the upper and lower front teeth.
