Cerebral hemorrhage is a relatively common condition at present. First of all, cerebral hemorrhage will interfere with the nerve tissue around the aneurysm, requiring patients to undergo various surgical examinations and treatments before they can recover. Generally, cerebral hemorrhage requires interventional treatment, and interventional treatment is surgical treatment. The main advantage of interventional treatment is that the technology is mature and safe. Compared with traditional craniotomy, it can reduce the patient's chance of physical injury.

Interventional surgery for cerebral hemorrhage

1. Advantages of interventional treatment of cerebral hemorrhage:

It can treat aneurysms in any part of the body, and only operates within the blood vessels without disturbing the tissue structure around the aneurysm, saving patients from the pain of craniotomy. It is particularly suitable for patients with serious illness, poor general condition, difficult aneurysm clipping and high surgical risks, such as posterior circulation aneurysms such as basilar artery bifurcation aneurysms. Intravascular interventional embolization surgery can shorten the postoperative recovery time, facilitate the functional training of postoperative patients, and play a positive role in preventing postoperative complications.

Endovascular embolization therapy technology is becoming increasingly mature, and studies have shown that its safety and overall efficacy have far exceeded traditional craniotomy. Critically ill patients in the acute phase often cannot tolerate traditional craniotomy methods, and some die while waiting for surgery.

Intravascular embolization can be performed in the acute phase without causing re-rupture and bleeding due to changes in aneurysm wall pressure. It can achieve immediate hemostasis and occlusion, and can simultaneously treat multiple intracranial aneurysms that cannot be treated with a single craniotomy. Its advantages are less trauma, greater safety, and only operations are performed within the blood vessels without disturbing the surrounding tissue structure, making it more easily accepted by patients and medical workers. Endovascular embolization treatment of cerebral aneurysms is a new trend in current development.

Scope of diagnosis and treatment: cerebral aneurysm, cerebral arteriovenous malformation, dural arteriovenous leak, cavernous sinus arteriovenous leak and refractory epistaxis, etc.

Surgical method: Puncture the femoral artery, insert a catheter and guidewire, and inject contrast agent to show vascular lesions (i.e. cerebral angiography); embolization treatment of intracranial aneurysms, including: controllable embolization technology with detachable coils; balloon-assisted embolization technology; stent-assisted embolization technology.

Interventional treatment of cerebral arteriovenous malformation (AVM): NBCA glue embolization; Onyx embolization (the latest embolization material with high operational safety). Interventional treatment of carotid cavernous fistula (CCF): detachable balloon embolization (economical, good effect); microcoil embolization; Onyx embolization. Embolization of dural arteriovenous fistula (DAVF): Onyx embolization (the best method currently). Interventional treatment of cerebral venous thrombosis (CVT): contact thrombolysis within the venous sinus; stent placement and angioplasty within the venous sinus.

2. Advantages of interventional treatment for cerebral ischemia:

The incision is small and the physical requirements are low, which is especially suitable for elderly people with multiple underlying diseases and poor physical conditions. The recovery is fast, and the patient can eat immediately after the operation. Generally, he can get out of bed and move around after lying flat for a few hours. The therapeutic effect is obvious, and the narrowed part of the blood vessel is immediately relieved during the operation to improve the blood supply to the brain. Combining drugs after the operation can reduce the incidence of cerebrovascular events. There are few complications; the patient suffers less pain and the cost is low. It has shown great advantages and application prospects in the treatment of ischemic cerebrovascular diseases.

Cerebral angiography can show the location and blood supply of intracranial and extracranial vascular lesions. It is the "gold standard" for diagnosing lesions such as stenosis or occlusion of cerebral arteries, congenital vascular defects, aneurysms and arteriovenous malformations. It is also the standard for preoperative evaluation of interventional treatment.
