The proliferation of anaerobic bacteria is a prerequisite for the treatment of anaerobic infections. In case of Clostridium perfringens myositis (i.e. gas gangrene), the muscles are extensively necrotic and must be removed. In severe cases, amputation may even be required. Necrotizing fasciitis is a rare anaerobic infection that often causes extensive necrosis of the fascia and skin. If not completely removed, it is often difficult to control the spread of the infection and may lead to death.

Antibiotics must be selected to be sensitive to anaerobic bacteria (see Table 6-5). Anaerobic bacteria are often resistant to aminoglycoside antibiotics. The antibacterial spectrum of lincomycin is similar to that of penicillin G and can be used if the patient is allergic to penicillin. Chloramphenicol is effective against almost all anaerobic bacteria, including Bacteroides fragilis, but the disadvantage is the risk of bone marrow suppression. Anaerobic bacteria have different susceptibilities to tetracycline, erythromycin, and chloramphenicol, and can rapidly develop resistance during treatment. Clindamycin is more effective than lincomycin for anaerobic infections, but like lincomycin, it can sometimes cause fatal pseudomembranous colitis. Among the current antibacterial drugs, metronidazole is the most effective, and is effective against all anaerobic bacteria including Bacteroides fragilis. Sharp et al. (1977) found that metronidazole is more effective than clindamycin and lincomycin. This drug is cheap and has no serious complications even with long-term use. Metronidazole can not only be taken orally (500 mg, three times a day) and by enema (1-2 g each time), but also has recently been developed in China as an intravenous preparation, 0.5% 100 ml, for intravenous drip 2-3 times a day. The role of oral metronidazole and intravenous metronidazole in preoperative preparation for colon surgery has proven that both methods can effectively reduce wound infection rates.

Hyperbaric oxygen therapy

Hyperbaric oxygen can increase the oxygen tension of tissues and inhibit the reproduction of anaerobic bacteria. This therapy is an important aspect in the treatment of Clostridium difficile infections, but many people are skeptical about its value in treating non-spore-forming anaerobic infections.

Hydrogen Peroxide

Hydrogen peroxide is an effective drug for treating wounds infected with anaerobic bacteria. The new oxygen it releases can kill anaerobic bacteria. Zinc peroxide paste can be used to treat some anaerobic infections, particularly Meleney ulcers.
