What are the treatments for gallbladder cancer?

The treatment of gallbladder cancer mainly includes surgery, radiotherapy, chemotherapy and traditional Chinese medicine. In the early stage, surgery was often advocated as the main treatment, but most of the gallbladder cancer patients seen clinically are in the late stage, so the current surgical resection rate is very low and the curative effect after resection is also very poor.

Although there has been considerable progress in biliary surgery, the diagnosis and treatment of gallbladder cancer have not improved much. Therefore, the key to curing gallbladder cancer is to make an accurate and qualitative diagnosis in the early stage and to implement correct treatment, including surgical treatment.

How to treat gallbladder cancer? The combination of Chinese and Western medicine can improve the efficacy of gallbladder cancer treatment, especially for patients with advanced gallbladder cancer, those with unsatisfactory surgical results and low chemotherapy sensitivity. If combined with traditional Chinese medicine treatment, it can improve the patient's general condition and prolong survival.

After surgery in the middle and late stages of the disease, Chinese medicine can be used as an auxiliary treatment to restore the patient's physical condition to a certain extent by strengthening the body, eliminating evil, replenishing qi, nourishing blood, and harmonizing the spleen and stomach.

The combination of chemotherapy and Chinese medicine can reduce the toxic side effects of chemotherapy drugs, improve the body's immune system, and enable patients to tolerate treatment. It also has a synergistic effect. The combination of Chinese medicine and radiotherapy can reduce the damage of radiotherapy, promote the recovery of normal body functions, strengthen the body, and improve the efficacy.

1. Surgical treatment

The main treatment for gallbladder cancer patients is surgery. However, most patients find that their tumors can no longer be removed or can only be removed as palliative treatment during surgery.

There are three general types of surgical procedures:

1. After a cholecystectomy for gallstones or acute cholecystitis, gallbladder cancer was unexpectedly found in the pathological section, and the lesion was limited to the serosal layer below the gallbladder wall. Most scholars believe that cholecystectomy is sufficient for such patients and there is no need for extended radical surgery. They also believe that even if surgery is performed to expand the scope of radical surgery, it may not necessarily change the survival rate and prognosis.

2. For patients who have been diagnosed with gallbladder cancer during surgery, whose lesions have invaded the serosal layer, with or without local metastasis, and who are still likely to undergo surgical resection, extended radical cholecystectomy may be considered. Local resection of liver tissue around the gallbladder bed can be performed at the same time as cholecystectomy, with a range of at least 2 cm. Lymph nodes around the gallbladder lymphatic drainage area such as the portal vein, hepatic artery, and extrahepatic bile duct should be cleared.

If the tumor invades the back of the pancreas, pancreaticoduodenectomy is also required. Some people even perform extended right anterior liver lobe and left internal liver lobe resection and portal vein resection, and reconstruction surgery, etc., in order to achieve a radical cure. However, the expansion of the scope of surgery can significantly increase the mortality rate of surgery, and it is still doubtful whether it can improve the treatment effect. It is generally believed that if gallbladder cancer has invaded the serosal layer, even if an extended radical surgery is performed, the effect will not be ideal.

3. Gallbladder cancer has spread to the bile duct and has multiple liver metastases. Radical surgery is no longer possible. Such patients can undergo some palliative surgery to relieve symptoms and improve the quality of life. If there is obstructive jaundice, bile duct drainage is required to relieve jaundice and skin itching; if pyloric obstruction occurs, gastrojejunostomy is performed.

2. Radiation therapy

Gallbladder cancer has atypical early symptoms, and the radical resection rate is low. The recurrence rate after extended radical resection is high and is the main cause of death. Therefore, surgery combined with radiotherapy is advocated. Gallbladder cancer is sensitive to radiotherapy to a certain extent. Surgery plus radiotherapy can prolong life and improve quality of life.
