What are the key points in the diagnosis and examination of colorectal cancer?

The diagnosis and examination of colorectal cancer has always been a difficult problem in clinical diagnosis and examination. Experts say that ileoscopy is generally preferred over small intestinal barium irrigation in the diagnosis of Crohn's disease at the terminal ileum. A British study observed the diagnostic value of small intestinal barium irrigation in patients with normal or no obvious abnormalities in ileocolonoscopy.

The researchers retrospectively reviewed all patients who underwent small-bowel barium irrigation after colonoscopy over a 7-year period. Patients with a previous diagnosis of inflammatory bowel disease and those with evidence of inflammatory bowel disease on colonoscopy were excluded.

Results showed that among 96 patients with normal ileoscopy and normal or no obvious abnormalities in colonoscopy, 3 had abnormalities in small-bowel barium enema. Two of these patients had abnormal terminal ileal biopsies, but the gross observation of the ileum seemed normal. Small-bowel barium enema helped establish the diagnosis of Crohn's disease.

In another patient, the signs on the barium enema examination supported the previously established diagnosis. Among the 47 patients who did not undergo ileoscopy because of normal or no obvious abnormalities in colonoscopy, the signs on the barium enema examination in one patient were consistent with the previously established diagnosis.

This study suggests that in patients with normal or unremarkable ileoscopy, terminal ileal biopsy, and colonoscopy, small-bowel barium enema is rarely necessary. It should be performed only when the index of suspicion of small-bowel pathology is very high. It is very important to perform a terminal ileal biopsy in patients suspected of having Crohn's disease even if the gross findings on ileoscopy are normal.
