Preoperative preparation for liver resection in patients with liver cancer

The key points of preoperative examination for liver cancer patients are:

(1) Detailed medical history and comprehensive physical examination to understand heart, lung, and kidney functions.

(2) Focus on checking liver function: ① When serum protein testing finds that total protein is lower than 60g/L, albumin is lower than 30g/L, or the white/globulin ratio is inverted, it indicates a poor surgical prognosis. ② Serum bilirubin may increase when liver cancer is combined with hepatocellular or obstructive jaundice, which should be listed as a contraindication for surgery. ② If the prothrombin time is still 4 seconds higher than the normal control after more than one week of intramuscular injection of vitamin K, it not only reflects liver dysfunction, but also indicates that large-scale blood transfusion during surgery may cause bleeding tendency. ④ If serum transaminase continues to increase, the risk of liver resection is greater, and surgery should be performed only after liver protection treatment is fully strengthened.

(3) Preoperative treatment: ① Give a high-protein, high-carbohydrate, and high-vitamin diet, especially for patients with chronic and long-term wasting diseases, who need to strengthen dietary treatment. ② Give a large amount of vitamins B, C, and K before surgery. Patients with prolonged prothrombin time or bleeding tendency should be given a larger dose of vitamin K. ③ For patients with low serum protein, albumin or plasma transfusion can be given, and patients with anemia can receive small amounts of blood transfusions multiple times. ④ If there is inflammatory lesions, antibiotics should be given 1 to 2 days before surgery.
