What are the stages of endometrial cancer

Clinically, endometrial cancer usually spreads outside the uterus and is confined to the pelvic cavity (the vagina and parauterine tissues may be involved, but the bladder and rectum are not involved). Tumors spread within the pelvic cavity and involve the bladder or rectum (with obvious mucosal involvement), or there is dissemination outside the pelvic cavity.

(I) Clinical staging of endometrial cancer (FIGO, 1971)

Stage I: The tumor is confined to the uterine body.

Stage Ia: uterine cavity length ≤ 8 cm.

Stage Ib: The length of the uterine cavity is 8 cm.

Stage II: The tumor has spread to the cervix.

Stage III: The tumor has spread outside the uterus and is confined to the pelvic cavity (the vagina and parauterine tissues may be affected, but the bladder and rectum are not affected).

Stage IV: The tumor has spread to the pelvis and involved the bladder or rectum (with significant mucosal involvement), or has spread outside the pelvis.

(II) Surgical and pathological staging of endometrial cancer (FIGO, 1988)

Stage I Stage Ia: The tumor is confined to the endometrium.

Stage Ib: The depth of tumor invasion is ≤1/2 of the muscular layer. Stage Ic: The depth of tumor invasion is 1/2 of the muscular layer.

Stage II Stage IIa: Only the cervical mucosal glands are affected.

Stage IIb: Cervical stroma is involved.

Stage III Stage IIIa: The tumor involves the serosa and/or adnexa and/or peritoneal cavity with positive cytology.

Stage IIIb: Vaginal metastasis.

Stage IIIc: Metastasis to the pelvic lymph nodes and/or para-aortic lymph nodes.

Stage IV Stage IVa: The tumor invades the bladder or rectal mucosa.

Stage IVb: Distant metastasis, including intra-abdominal and/or inguinal lymph node metastasis.
