Melanoma of eyelid

Disease Overview Eyelid pigmented nevus is a congenital flat or raised lesion on the eyelid with clear boundaries and composed of nevus cells. It is usually present at birth, but rarely occurs during adolescence. The initial growth is faster, and then the growth is slower. Some will atrophy on their own and gradually become still in adulthood.

Clinical manifestations

Eyelid pigmented nevus can be divided into

① Junctional nevus, which is generally flat and consistent brown in color, and the nevus cells are located at the junction of the epidermis and dermis. There is a low-grade malignant tendency.

② Intradermal nevus, the most common, is generally raised and sometimes papilloma-like. Pigment is minimal and, if present, is brown to black. Nevus cells are entirely within the dermis and may not have malignant tendencies.

③Compound nevus, usually brown, is composed of the first two types of components combined together. There is a tendency towards low-grade malignancy.

④ Blue nevus, which is generally flat and has pigment almost from birth, and is blue or slate gray. No malignant trend.

⑤ Congenital oculocutaneous melanocytosis, also known as nevus of Ota, is a blue nevus surrounding the eye socket, eyelid and eyebrow skin. It is common in Orientals and blacks and has no malignant tendency. If it occurs in white people, it has a malignant tendency.

Treatment

If there are no signs of malignancy such as rapid enlargement, darkening, ulceration and bleeding, no treatment is necessary. If excision is necessary for cosmetic reasons, it must be complete and thorough, otherwise the remaining mole cells may be stimulated by the surgery and become malignant.
