Cervical cancer patients are becoming younger and younger. What details should we know about prevention?

The incidence and mortality of cervical cancer in my country have been on the rise in recent years. In 2013, the incidence of cervical cancer was 10.3/100,000 and the mortality was 2.6/100,000, of which the incidence in urban population was 10.1/100,000 and the incidence in rural population was 10.5/100,000. Cervical cancer has become the second most common malignant tumor among young and middle-aged women in China, with 131,500 new cases of cervical cancer each year, accounting for more than 28% of the global cases. At the same time, about 50,000 patients die from the disease every day. Clinical cases show that cervical cancer patients are becoming younger and younger.

Detail 1: Cervical cancer cannot be ignored during pregnancy

Cervical cancer is rare during pregnancy, but it is the most common malignant tumor during pregnancy. The treatment of cervical cancer during pregnancy is difficult for clinicians and requires a multidisciplinary team including gynecological oncologists, radiation oncologists, oncologists, obstetricians and pathologists. The treatment should be standardized, individualized and humane in combination with the wishes of the patient and her family.

Because the physiological functions and immune system of pregnant women are in a special state, many oncologists speculate that pregnancy will aggravate the progression of invasive cervical cancer. Cervical cancer during pregnancy makes clinicians face a difficult choice between two treatment methods: terminating the pregnancy and then treating cervical cancer, or preserving the fetus and giving treatment until delivery.

Traditionally, it is believed that once the diagnosis is confirmed, the pregnancy should be terminated as soon as possible before cervical cancer treatment. However, in recent years, it has been found that pregnancy does not accelerate the progression of cervical cancer. For cervical cancer diagnosed after 16 weeks of pregnancy, delaying treatment to wait for fetal lung maturity has not been shown to have an adverse effect on maternal prognosis.

Detail 2: Don’t be too pessimistic. The mortality rate of cervical cancer is decreasing year by year.

For some cervical cancer patients, the three traditional treatment methods of surgery, chemotherapy and radiotherapy cannot achieve satisfactory treatment results. In particular, drug resistance in cervical cancer treatment is a major difficulty in clinical treatment. Finding a reasonable and effective treatment for cervical cancer is an urgent problem to be solved in all medical work. Now more and more cervical cancer patients choose minimally invasive interventional treatment.

Detail 3: The role of cervical cancer vaccine cannot be ignored

In recent years, the incidence and mortality of cervical cancer in my country have been on the rise, and the trend is becoming younger. Some regions in my country are relatively in the leading position in cervical cancer prevention, and their advanced experience is worth learning from. With the launch of 2-valent and 4-valent vaccines, my country's primary prevention of cervical cancer will be further developed. However, we should also realize that the promotion of HPV vaccines is a long-term task, and there is still a long way to go to prevent cervical cancer.
