There are three main surgical methods for gastric cancer. The first method is radical resection, that is, ESD under digestive endoscopy, which is a relatively minimally invasive treatment that can completely remove the lesion and is suitable for patients with early gastric cancer without lymph node metastasis. The second method is palliative resection, which is suitable for patients with early gastric cancer with lymph node metastasis or patients with advanced gastric cancer at an earlier stage. The third method is short-circuit surgery, which is suitable for patients with relatively advanced gastric cancer.

Gastric cancer is a disease that everyone is familiar with. The occurrence of this disease has seriously affected people's normal life. The treatment plan for this disease mainly depends on the tumor stage. If the preoperative stage is before stage ⅢA (excluding stage ⅢA), surgical treatment can be the first choice. For stage ⅢA and later, preoperative chemotherapy or chemoradiotherapy can be selected, and surgical treatment can be performed after the tumor is reduced and the tumor is downstaged. So, what are the surgical methods for gastric cancer?
Radical gastrectomy is a minimally invasive surgery for traditional open radical gastrectomy, which can relieve the pain of patients to the greatest extent. Traditional radical gastrectomy is to make a 20~30cm incision in the upper abdomen of the patient. Regardless of whether it is aesthetically pleasing or not, the patient is severely injured during the operation and recovers slowly. Laparoscopic radical gastrectomy only requires a small incision of 6~8 cm in the patient's abdomen, and a hole of +40.5 cm can be used for surgery. The patient has less trauma, so there are also advantages of much faster recovery after surgery, and the patient can get out of bed and move around after 2 days, gastrointestinal motility will recover in 3 days, and discharge can be completed in a week.
For patients, the postoperative pathological section report also showed that laparoscopic radical gastrectomy achieved the same effect as open surgery in lymph node dissection. Therefore, it was recognized and accepted by patients and their families, and this treatment method was welcomed by more and more patients in the medical field.
Radical gastrectomy is divided into radical or extended radical gastrectomy according to the extent of tumor resection and surrounding lymph nodes. Radical resection should be based on the distal 2/3 or 4/5 of the stomach as the primary lesion, the size of the complete omentum, the first segment of the duodenum and local lymph nodes and organs infiltration, and no cancer residue at the cut end of the stomach or duodenum.
Radical gastrectomy is recommended for patients with tumors located in the gastric antrum and distal lesser curvature. In addition to the above, the entire stomach or adjacent transverse colon, left lobe of liver, spleen, pancreatic tail, left cardia and parasplenic lymph nodes should be removed. In the advanced stage of the disease with deep lymph node metastasis or gastric cancer, diffuse invasive cancer should be considered for extended radical resection.
