What is the function of the ileum ?

The main physiological function of the ileum is to digest and absorb food and move the chyme forward. The alkaline intestinal fluid secreted by the ileal mucosa contains a variety of enzymes, which can break down the chyme into glucose, amino acids and fatty acids, which are then absorbed through the capillaries of the intestinal mucosa and reach the liver through the portal vein. The ileal mucosa also has the function of absorbing and diffusing ions. When the ileal mucosa is in extensive contact with urine for a long time, the chloride ions, hydrogen ions, ammonium ions and nitrogen-containing substances in the urine can be absorbed into the blood, resulting in hyperchloremic acidosis and hypokalemia. The ileocecal valve at the junction of the terminal ileum and cecum acts as a sphincter. In continent cecal cystometry, the ileocecal valve is used in part to prevent the backflow of urine.

The jejunum and ileum are collectively called the small intestine, which is 3 to 5 meters long. The ileum is 3/5 of the distal segment of the small intestine, located in the lower right part of the abdominal cavity, and part of the ileum is also in the pelvic cavity. The wall of the small intestine is covered by a serosa, and beneath the serosa is a muscular layer. The muscle fibers are arranged in longitudinal rows on the outside and circular rows on the inside. The muscle layer is thicker in the proximal segment, gradually thins toward the distal segment, and thickens again near the ileocecal region. The mucosal layer relaxes and forms annular folds.

The root of the small intestinal mesentery starts from the left side of the 1st or 2nd lumbar vertebra, runs obliquely to the lower right, and ends in front of the right sacroiliac joint. The total length is about 15cm. The small intestinal mesentery and its root are composed of two layers of peritoneum, which contain blood vessels, nerves and lymphatic tissue. Because the length of the small intestine much exceeds the length of its mesentery root, and the distance between the mesentery root and the intestinal tube is longer in the middle and shorter at both ends, the small intestinal mesentery is fan-shaped and has many folds. The ileum mesentery is characterized by more adipose tissue and smaller and denser vascular arcades.
