Hematogenous metastasis of liver cancer in late stage: Intrahepatic hematogenous metastasis occurs earliest and is the most common, and can invade the portal vein and form tumor thrombi. Tumor thrombi that fall off in the liver can cause multiple metastatic lesions. Tumor thrombi that block the main trunk of the portal vein can cause portal hypertension and refractory ascites. After invading the hepatic vein, liver cancer cells can enter the systemic circulation and metastasize outside the liver, with the highest metastasis rate in the lungs. It can also metastasize to various parts of the body, with the adrenal glands, bones, kidneys, brain and other organs being more common. Hematogenous metastasis is common in hepatocellular carcinoma.

Liver cancer cells can easily reach the lungs through the bloodstream. When lung metastasis occurs, the patient may experience chest pain, cough, hemoptysis, shortness of breath and other symptoms. Liver cancer can also metastasize to the pleura, forming pleural effusion. The patient feels breathless and the pleural effusion is bloody, and cancer cells can be found in it. When liver cancer patients find respiratory symptoms such as chest pain and cough, they should highly suspect lung metastasis of liver cancer. They can take chest X-rays and CT scans of the patients. If there is pleural effusion, thoracentesis should be performed to extract pleural effusion for testing.

Late-stage lymph node metastasis of liver cancer: Local metastasis to the hepatic hilar lymph nodes is the most common, and it can also metastasize to the supraclavicular, para-aortic, pancreatic, splenic and other lymph nodes. Lymph node metastasis is the most common metastasis of cholangiocarcinoma. Lymph node metastasis only accounts for 12.6% of the total metastasis.

Direct infiltration of liver cancer in the late stage: Liver cancer generally rarely directly infiltrates adjacent organs, but occasionally it can directly spread and infiltrate adjacent tissues and organs, such as the diaphragm, stomach, colon, omentum, etc.

Late-stage implantation and metastasis of liver cancer: It occurs occasionally, such as implantation in the retroperitoneum to form bloody ascites. Women may also have ovarian metastatic cancer.
