Which department should I go to for prostate treatment? What examinations should I do for prostate treatment?

If you want to check your prostate, you should go to the andrology department, because prostate disease is a andrological disease. Only after completing andrology-related examinations can you know whether your prostate is likely to have a lesion. Generally, it is difficult to confirm whether you have prostate disease by just doing a CT scan. You should still do a more comprehensive physical examination, mainly to avoid misdiagnosis.

Prostate examination is mainly performed through rectal examination. During the examination, attention should be paid to the size, shape, hardness, presence of nodules, tenderness, fluctuation, and the condition of the median groove of the prostate. If chronic prostatitis is suspected, prostate massage is required to obtain prostate fluid for bacterial culture and laboratory examination. This technique can also be used as a treatment method.

Abnormal results: When prostate cancer grows in the capsule, it is difficult to diagnose with CT. It can only be diagnosed when it breaks through the capsule and infiltrates into the surrounding fat tissue. It manifests as an irregular prostate contour and uneven density. The anterior rectal wall and bladder wall may be infiltrated, and the seminal vesicle angle disappears. CT can also detect lymph node metastasis and pelvic metastasis. CT is helpful for the staging of prostate cancer.

On MRI, prostate hyperplasia mainly occurs in the central area, so T2WI shows enlargement of the central area. Prostate cancer mostly occurs in the peripheral area, resulting in a limited low signal or slightly low signal nodular shadow in the peripheral area on T2WI. Therefore, MRI has more diagnostic value than CT. However, when the MRI manifestations of prostate cancer are atypical, diagnosis is still difficult.

People who need to be checked: middle-aged and elderly men who often have difficulty urinating and sometimes feel that their urine cannot be emptied. They have frequent urination due to the inability to empty the bladder, especially those with symptoms such as increased number of nocturia, difficulty urinating, significantly reduced urine volume, and poor urine flow.

1. During CT examination of the bladder, the bladder needs to be properly filled to distinguish the bladder wall from the inner cavity. Drink plenty of water and do not urinate to fill the bladder.

2. Empty your urine before undergoing a prostate digital examination.

3. When doing prostate fluid culture, you should abstain from sex for more than 5 days before collecting the prostate fluid. Otherwise, it will affect the test results.

4. Antibiotics should be discontinued before collecting prostate fluid culture specimens. This is because after entering the body, antibiotics can reach various tissues and organs in a short period of time, thereby inhibiting the growth and reproduction of bacteria and other microorganisms in the specimen in the culture medium, resulting in false negative results and missed diagnosis of certain pathogens.

Therefore, when patients visit the doctor, if they are receiving antibiotic treatment, whether it is for the treatment of prostatitis or other organ infections, they should generally stop taking the medicine for 3 days before collecting urine, prostatic fluid and other specimens, as well as routine examination and isolation and culture of pathogens. Of course, in some special cases, such as acute prostatitis or other special reasons that cannot wait, this point does not need to be taken into account.
