Introduction to drug factors and causes of common infusion reactions

There are four main situations

1.1 Quality of large infusions. Quality problems can easily occur due to contamination of raw materials, contamination during the production process, or incomplete disinfection. In recent years, the implementation of GMP has been strengthened, and quality problems generally do not occur in infusions. However, during transportation, storage, and use, if collisions occur and small cracks appear, or if the bottle mouth becomes loose and leaks, large infusions, especially those containing sugar, are very likely to deteriorate under the above circumstances.

1.2 Pyrogen superposition. Intravenous administration and combined medication are more common. When the pyrogenic substances of various drugs are added together, it is possible that they exceed the threshold and cause a pyrogenic reaction.

1.3 Infusion reactions caused by superposition of particles. Frequent combined use of drugs results in an increase in insoluble particles and a greater chance of infusion reactions. Improper operation, infusion equipment, and infusion environment are also important causes of particle superposition. Such as incomplete dissolution of powder injections, changes in the pH of the infusion, improper operation during the preparation process, etc.; multiple punctures of the rubber stopper when adding drugs cause rubber crumbs to fall into the bottle; incorrect or no disinfection when cutting the ampoule, and the ampoule is broken as soon as it is sawed open, then due to the negative pressure inside the ampoule, a large amount of glass particles will be sucked into the drug solution.

1.4 Improper diluent selection. Chinese herbal injections such as Xuesaitong, Shengmai, and Ginkgo Biloba are more likely to experience infusion reactions. The reason is that Chinese herbal medicines have complex ingredients and different manufacturers have different preparation processes, which leads to great differences in the extraction of effective ingredients and removal of impurities. Some ingredients such as pigments, tannins, starch, proteins, etc. exist in the drug solution in colloidal form. After the drug is combined with the infusion, it will undergo oxidation and polymerization, or the pH change will cause the precipitation of bioalkalis, saponins, etc., resulting in a large number of insoluble particles and other allergenic substances, which increases the incidence of infusion reactions.
