Anal leakage can cause repeated pus discharge, anal pain, and itching. There are many clinical classifications of anal leakage. Introduction Anal fistula is mostly caused by the poor drainage of pus after anal abscess rupture, resulting in residual toxins and sequelae. Anal fistula consists of three parts: the primary internal opening, the duct and the secondary external opening. It is characterized by recurrent pus discharge, itching and pain around the anus. It is divided into simple anal leakage, complex anal leakage, low anal leakage and high anal leakage. For the syndrome of damp-heat descending, the treatment should be to clear away heat and dampness, and the prescription can be Er Miao Wan combined with Bi Shan Shen Shi Tang with modifications; for the syndrome of yin deficiency and evil retention, the treatment should be to nourish yin and clear away heat, and the prescription can be Qing Hao Bie Jia Tang with modifications. Surgery is an effective method to completely cure anal fistula. There are two types of surgery: excision therapy and incision and hanging thread therapy. Excision therapy is suitable for low-position anal fistula, while incision and hanging thread therapy is suitable for high-position anal fistula. Accurately finding the primary internal opening is the key to a successful operation. If the anorectal ring is cut off during the operation, anal incontinence will occur. 2. Clinical classification (1) Simple anal fistula: There is only one external opening in the skin near the anus, and the duct leads directly into the anal crypt. If the inner and outer ports are connected, it is called internal and external leakage, also called complete leakage; if there is only an external port and no internal port, it is called external leakage, also called external blind leakage; if only the internal port is connected to the pipe and there is no external port, it is called internal leakage, also called internal blind leakage. (ii) Complex anal fistula refers to two or more openings inside or outside the anus, or the duct passes through more than two gaps, or there are many ducts with branches growing horizontally, or the duct grows around the anus in a horseshoe shape. (III) Low-position anal fistula refers to an anal fistula in which the duct is deep below the external sphincter and the internal opening is in the anal recess. (IV) High-position anal fistula refers to an anal fistula that passes through the deep layer of the external sphincter, or the internal opening of the anal fistula is above the dentate line. |
