What are the symptoms of urinary retention?

1. Main symptoms

1. Urinary retention occurs suddenly. The bladder is full of urine and cannot be discharged. The patient suffers from unbearable distension and pain, tossing and turning, and sometimes some urine overflows from the urethra, but it cannot relieve the pain in the lower abdomen.

2. Chronic urinary retention is often manifested as difficulty urinating, frequent urination, a feeling of incomplete urination, and sometimes urinary incontinence. Although a small number of patients do not have obvious symptoms of chronic urinary retention and obstruction, they often have obvious upper urinary tract dilatation, hydronephrosis, and even symptoms of uremia, such as physical weakness, anemia, urine odor on breath, lack of appetite, nausea and vomiting, anemia, increased serum creatinine and urea nitrogen, etc.

2. Clinical Diagnosis

Diagnosis is by history and symptoms and signs of a full bladder but no urine flow or incomplete emptying. After urine retention is discovered through inspection and percussion of the suprapubic area, it is further confirmed through B-ultrasound examination and catheterization.

Etiological diagnosis relies on asking about the medical history of urinary tract infection, urinary stone excretion, urethral injury, prostate disease, central nervous system infection and diabetes, combined with symptoms, signs, bladder X-ray examination, B-ultrasound and urethra and cystoscopy to find out the cause of urinary retention.

3. Differential Diagnosis

Urinary retention should be differentiated from anuria, which refers to renal failure or complete upper urinary tract obstruction, resulting in an empty bladder and no urine.

The causes of urinary retention can be divided into three categories:

1. Urethral stenosis and obstruction: urethral obstruction caused by urethral inflammation, edema or stones, urethral stenosis, trauma, prostatic hyperplasia or tumor, acute prostatitis or abscess, bladder tumor, etc.

2. Bladder disease or dysfunction: bladder stones, inflammatory scars, tumors, bladder neck hypertrophy, etc., which can narrow or obstruct the urethral opening;

3. Neurological factors: central nervous system diseases caused by various reasons and autonomic nerve damage caused by diabetes, etc.

5. Treatment Methods

Acupuncture at the Yinlingquan, Zusanli, Sanyinjiao, Guanyuan, Zhongji, Shuidao and other acupoints can help urine be discharged in a shorter period of time. Or use Chinese medicine massage techniques, use your palms and fingers to gently press the front wall and bottom of the patient's bladder dozens of times, which can enhance the contraction function of the bladder smooth muscle and often promote urination. You can also use your fingers to apply even and moderate force to press the Guanyuan, Qihai, Zhongji and other acupoints, and at the same time ask the patient to exert force to force out the urine. In addition, you can also use medicines, such as Chinese herbal medicines that promote diuresis and relieve stranguria, and you can also use hot compresses on the lower abdomen to treat urinary retention. If the above methods do not work, catheterization can be used to drain urine out of the body and relieve pain.
