There is an asymptomatic period of more than 48 hours (also called the Baudet latent period) between trauma and splenic rupture and bleeding. If it exceeds 48 hours, it can be considered as delayed splenic rupture. Clinical manifestations 1. stomach ache: A ruptured spleen causes abdominal pain and tenderness. It is persistent and generally not very severe. Central rupture and subcapsular rupture are difficult to detect and diagnosis is delayed because the capsule is intact and there are no obvious clinical signs of internal bleeding. Of the clinical cases of spleen rupture, about 85% are true ruptures. The patient bleeds heavily and can quickly go into shock, or even die if not treated in time. 2. Rebound pain, muscle tension: The bleeding in the abdominal cavity acts like an irritant, causing pain, and the abdominal muscles feel stiff to the touch due to reflex contraction. Intraperitoneal bleeding after spleen rupture may cause peritoneal irritation signs, but they are not severe. 3. If bleeding is gradual: There may be no symptoms until the body's blood supply is so depleted that blood pressure drops or oxygen is not delivered to the brain and heart. This condition is an emergency and requires immediate blood transfusion to maintain adequate blood circulation and surgery to stop the bleeding. Without these measures, the patient may go into shock and die. The pain caused by spleen rupture may be accompanied by nausea and vomiting if it is an incomplete rupture.
