Surgical treatment of cholangiocarcinoma The most effective treatment for bile duct cancer is surgery. Depending on the location of the lesion, the surgical treatment method is also different. Here are a few examples: (1) Resection of the quadrate lobe of the liver or part of the right anterior lobe, resection of the hilar bile duct and extrahepatic bile duct, and choledochojejunostomy. Applicable to common hepatic duct cancer or confluent bile duct cancer. (2) Resection of the quadratic lobe or left hemihepatectomy and resection of the portal bile duct and extrahepatic bile duct, and choledochojejunostomy. Radiotherapy has a certain effect on advanced bile duct cancer because it can cause cancer cells to degenerate and necrotize, and inhibit their growth, which can prolong the survival of patients with advanced bile duct cancer. 3. Chemotherapy for cholangiocarcinoma Chemotherapy for bile duct cancer is used as an auxiliary means after surgery or in cases where the cancer cannot be removed. It can be divided into intraoperative radiotherapy, postoperative localized radiotherapy and staged internal irradiation. Radical dose radiotherapy has a certain effect on advanced bile duct cancer because it can cause cancer cells to degenerate, necrotize and inhibit their growth, which can prolong the survival of patients with advanced bile duct cancer. Todoroki reported that the 3-year survival rate of bile duct cancer lesion resection plus radiotherapy was 10%, while that of those without radiotherapy was 0%. The general radiation dose is 40 to 50 GY. Intraoperative radiation is the use of an electron beam generated by a cyclotron to give a radiation dose of 20 to 30 GY after the lesion is removed. But in general, chemotherapy for bile duct cancer is less effective. Therefore, radiotherapy is completely unnecessary. Regular check-ups are sufficient. Without TCM treatment, the prognosis of bile duct cancer is extremely poor. The average survival of the surgical resection group is generally 13 months, and few survive for 5 years. If only internal or external drainage of the bile duct is performed, the average survival is only 6 to 7 months, and rarely exceeds 1 year. |
