What are the conditions for unilateral total resection of thyroid cancer

Thyroid cancer is a malignant tumor that originates from the thyroid follicular epithelium or parafollicular cells. Once a patient is diagnosed with thyroid cancer, he or she should seek medical treatment for surgical resection in a timely manner. For patients with unilateral thyroid cancer, unilateral total resection can be performed if the surgical indications are met. There are no conditions that must be met.

1. No distant metastasis: If the patient has no metastasis to distant organs, such as lung metastasis, liver metastasis, bone metastasis, etc., only cervical lymph node metastasis or lung metastasis, and the tumor size does not exceed 4cm;

2. No local tumor infiltration: Unilateral thyroidectomy can be performed if the surgical indications are met, and there are no conditions that must be met;

3. Large tumors on both sides of the thyroid gland: If the patient's tumor is located in both lobes of the thyroid gland and is larger than 4 cm, or the tumor is located in the isthmus of the thyroid gland and cannot be connected to the local trachea, esophagus, and recurrent laryngeal nerve, it is difficult to remove the tumor during thyroidectomy. In this case, unilateral total resection should be considered;

4. The patient has no cardiopulmonary dysfunction: If the patient has normal cardiopulmonary function, can tolerate unilateral total resection, and has no serious cardiopulmonary disease, such as heart failure, respiratory failure, etc., unilateral total resection can be performed;.

5. Patients are allergic to iodine: If the patient is allergic to seafood or other foods or has a positive iodine-131 test before surgery, it is necessary to avoid using iodine-containing contrast agents during unilateral total resection surgery to avoid allergic reactions;

6. The patient is in the stable stage of the disease: If the patient has no recurrence of thyroid hormone suppression treatment within 2 years after thyroid cancer surgery, it can be judged as a stable state, and unilateral total resection surgery can be performed at this time.

In addition to the above conditions, for elderly patients, such as those with comorbidities such as diabetes, hypertension, cerebral infarction, etc., the operation time should be increased according to the patient's condition during surgical treatment to reduce the surgical risk.
