Some of this hematuria is intermittent like kidney cancer, and the intervals vary in length, ranging from several weeks to several months; some bladder tumor patients have hematuria accompanied by blood clots, indicating that the tumor has invaded the larger blood vessels of the bladder mucosa and the amount of bleeding is relatively large. When accompanied by blood clots, symptoms of urination pain and urgency will appear during urination.

Most bladder tumors present painless gross hematuria or microscopic hematuria as the first symptom. Patients present with intermittent and continuous hematuria, sometimes accompanied by blood clots. Clinically, intermittent painless gross hematuria is considered a typical symptom of bladder tumors. The amount of bleeding and the duration of hematuria are related to the malignancy, size, range and number of the tumor, but they are not necessarily proportional. Sometimes when gross hematuria occurs, the tumor is already very large or in the late stage; sometimes a very small tumor will cause a large amount of hematuria. Because hematuria is intermittent, when hematuria stops, it is easy for patients to ignore it, mistakenly believing that the disease has disappeared and not conducting further examinations in a timely manner.

Hematuria is often painless and intermittent, and can be relieved or stopped on its own, which can easily give the person the illusion that the disease has been cured or improved. A small number of bladder cancer patients may experience symptoms such as increased urination frequency, urgency and pain when urinating, as if they have cystitis, and antibiotics are completely ineffective. There are also a few bladder cancer patients who do not have macroscopic hematuria, but only show excessive red blood cells when examining the urine under a microscope, which is called microscopic hematuria.
