It can be said that the three indicators of hepatitis B surface antigen (HBsAg), hepatitis B e antigen (HBeAg), and hepatitis B core antibody (anti-HBC) are positive. The positive results of these three indicators often indicate that the virus replication in the body is relatively active, but whether it has caused serious liver cell damage depends on the liver function test results and the patient's subjective symptoms.

General indications include: ① For HBeAg-positive patients, HBV-DNA ≧105 copies/ml (equivalent to 20000 IU/ml); for HBeAg-negative patients, HBV-DNA ≧104 copies/ml (equivalent to 2000 IU/ml); ② ALT ≧2×ULN; if treated with IFN, ALT should be ≦10×ULN and serum total bilirubin should be ﹤2×ULN; ③ ALT ﹤2×ULN, but liver histology shows KnodellHAI ≧4, or inflammatory necrosis ≧G2, or fibrosis ≧S2.

For patients with persistent HBV-DNA positivity who do not meet the above treatment criteria but meet one of the following conditions, antiviral treatment should also be considered: ① For patients with ALT greater than ULN and age ≥40 years, antiviral treatment should also be considered (III); ② For patients with persistently normal ALT but older age (≥40 years), close follow-up and preferably a liver biopsy should be performed; if liver histology shows KnodellHAI≧4, or inflammatory necrosis≧G2, or fibrosis≧S2, antiviral treatment should be actively given (II); ③ For patients with evidence of disease progression (such as enlarged spleen) found through dynamic observation, liver histology examination is recommended and antiviral treatment should be given if necessary (III).

Before starting treatment, ALT elevation caused by drugs, alcohol or other factors should be ruled out, and ALT transiently normalized after the use of enzyme-lowering drugs should also be ruled out. In some special diseases such as liver cirrhosis or patients taking biphenyl derivative drugs, the AST level may be higher than ALT. In this case, the AST level can be used as the main indicator.
