What are the nursing measures to prevent pressure sores

Pressure sore. Many people don’t know what this disease is at first. In fact, it is a kind of disease caused by long-term pressure on the patient’s local tissue, which affects blood circulation, causes ischemia, hypoxia and malnutrition of the local subcutaneous tissue of the skin, resulting in tissue ulceration and necrosis. It is a disease that causes great pain to patients and may be life-threatening in severe cases, so it is very necessary for us to understand it and prevent it.

1. Reasons:

(I) Mechanical factors:

1. Pressure:

Unable to change body position independently and improper plaster fixation padding.

2. Friction:

The semi-recumbent posture is incorrect, the turning method is incorrect, the sheets and clothes are wrinkled, the wheelchair slides down, and the toilet is used improperly.

3. Shear force.

1. Nursing assessment:

1. Identify high-risk groups: those who are comatose, paralyzed, and elderly; those who are obese, weak, and malnourished; those who take sedatives; those who are restrained, anesthetized, have edema, fever, pain, incontinence, and are in plaster.

2. Perform local assessment of skin pressure.

(II) Nursing diagnosis:

There is a risk of shrinkage of skin integrity. It is associated with impaired body movement, urinary incontinence and local skin moisture.

(III) Develop a nursing plan:

Six diligences (frequent observation, frequent turning over, frequent cleaning, frequent tidying up, frequent replacement, and frequent massage) are strictly enforced during shift handover.

1. Avoid long-term pressure on local tissues. Observe and turn over frequently.

(1) Change lying position frequently to intermittently relieve the pressure on local tissues;

(2) Protect bony prominences and support body voids;

(3) Correctly use plaster bandage and splint fixation;

(4) Apply pressure-reducing dressings and mattresses.

2. Avoid shear force and friction.

3. Avoid stimulation from moisture and friction. Clean, organize and replace frequently.

4. Improve the body's nutritional status. Positive nitrogen balance.

6. Encourage patients to move around.

7. Increase the health knowledge of patients and their families.
