1. If the scar is extensive, the excision range should also bypass the corners of the mouth to the upper lip, so that the lower lip after skin grafting can be pulled upward and not droop. Then take a medium-thickness or full-thickness skin graft of the corresponding size, transplant it to the wound surface, pack it, cover it with a small amount of gauze, and fix it to the cheeks with adhesive tape. It is advisable to use nasogastric feeding after surgery to prevent food from flowing into the wound.

2. Local flaps can be used to repair localized scars on the lower lip according to different situations.

(1) Localized eversion of the lower lip: The lip edge can be cut open, and the scar tissue can be removed in a triangular or "V" shape. Then the lip edge incision can be extended, and the subcutaneous tissue of the lower lip can be separated, so that the skin flaps on both sides are pulled together and sutured. To prevent secondary contracture, a "Z"-shaped skin flap can be made to change its linear scar. If the lower lip mucosa appears redundant, the incision of the lower lip edge can be extended, and a suitable location can be selected to remove part of the mucosa. However, it is not advisable to remove it at the skin suture to avoid a sunken deformity. If the skin on one side is loose, it can be removed in a fusiform shape together with the mucosa, and then the skin on one side can be transferred to the other side to repair the eversion and avoid a linear scar.

(2) If the lower lip is completely everted: If the skin of the lower jaw is loose, a skin flap curved toward the chin can be designed at the lower jaw outside the affected side of the mouth corner and transferred to the lower lip to repair the eversion.

(3) Eversion caused by localized skin defect on one side of the lower lip: A skin flap can be designed at the corner of the mouth, and the lip edge can be cut open to fully free the subcutaneous tissue and completely loosen the adhesion of the red lip. Then the flap can be pulled and fixed at the corner of the mouth, and the skin flap at the corner of the mouth can be transferred to the defect of the lower lip for repair.
