The pressure in the bile duct will further increase, and purulent bile (including bacteria and berry factor) can flow back into the blood through the bile duct capillaries, causing sepsis, and may also cause erosion and rupture of the bile duct wall, and even cause atrophy of the bile duct, hepatic artery and portal vein, leading to massive bile duct bleeding.

③ Obstruction and infection lead to liver cell damage, and may even cause liver cell necrosis and formation of biliary liver abscess. Repeated attacks of bile duct inflammation can also lead to biliary cirrhosis. ④ When gallstones are impacted in the ampulla of the common bile duct, they can cause acute and/or chronic inflammation of the pancreas, which is called biliary pancreatitis. The clinical manifestations of extrahepatic bile duct stones depend on the presence or absence of infection and obstruction. Usually there are no symptoms. However, when stones block the bile duct and secondary infection occurs, the typical clinical manifestation is Charcot's triad, namely abdominal pain, chills, high fever and jaundice.

① Abdominal pain: occurs under the xiphoid process and in the right upper abdomen, mostly colic, with paroxysmal attacks, or persistent pain that worsens in paroxysms, and may radiate to the right shoulder and be accompanied by nausea and vomiting. ② Chills and high fever: After infection secondary to bile duct obstruction, the pressure in the bile duct increases, and the infection spreads retrogradely along the bile duct. Bacteria and toxins enter the hepatic sinusoids through the bile duct capillaries to the hepatic veins, and then enter the systemic circulation to cause systemic infection. About 2/3 of patients may experience chills and high fever during the course of the disease, generally manifesting as remittent fever, with the body temperature reaching 39~40℃.
