Basic functions of kidneys

(1) Production of urine. When blood flows through the kidneys, most of the plasma components except cells and large molecular proteins are filtered into the renal capsule through the filtration membrane composed of the glomerular capillary endothelium, basement membrane and podocyte slit membrane to form primary urine. In the process of flowing through different segments of the renal tubules, the urine is concentrated and diluted to eventually form final urine, which flows into the renal pelvis and is excreted from the body.

(2) Excretion of metabolic products. The body produces a variety of wastes during the metabolism process. Most of the metabolic wastes - including more than 100 kinds of metabolic wastes and toxic substances represented by urea nitrogen, creatinine, uric acid, etc. - enter the kidneys through the blood, are filtered by the glomeruli or secreted by the renal tubules, and are excreted from the body with urine.

(3) Maintain body fluid, electrolyte balance and body fluid acid-base balance. In the kidneys, water and electrolytes in the blood are filtered into the primary urine through the glomeruli; the water and electrolytes in the primary urine are reabsorbed in different proportions when flowing through different segments of the renal tubules, and some electrolytes will be secreted into the tubular lumen. The kidneys maintain the balance of water, electrolytes, acids and alkalis in the body through the urine concentration and dilution process, thereby maintaining the stability of the internal environment.

(4) Endocrine function:

① Secrete renin, prostaglandins, and kinins. Through the renin-angiotensin-aldosterone system and the kinin-bradykinin-prostaglandin system. System to regulate blood pressure.

② Secrete erythropoietin. Stimulate bone marrow hematopoiesis.

③ Activate vitamin D3. Regulates calcium and phosphorus metabolism.

④ The degradation site of many endocrine hormones, such as insulin, gastrointestinal hormones, etc. When renal function is impaired, the T1/2 of these hormones is significantly prolonged, causing metabolic disorders.

⑤ Target organs of extrarenal hormones. Such as parathyroid hormone, calcitonin, etc. Can affect and regulate kidney function.
