How to solve the problem of liver enlargement caused by drug-induced hepatitis

If drugs cause hepatitis and the liver becomes enlarged, you must pay attention to the treatment and follow the doctor's advice on the use of any drugs. You should also pay attention to bed rest, cooperate with some diet therapies, and make sure to supplement enough protein and calories.

1. Immediately discontinue use of drugs associated with or suspected of liver damage.

2. Rest in bed and adhere to diet therapy (same as diet for viral hepatitis) to supplement sufficient calories, protein, and multiple vitamins such as vitamin C, E, B, etc. to facilitate liver cell repair and regeneration.

3. For severe jaundice, intravenous drip of glucose, vitamin C, and Chinese medicine Yinzhihuang should be given to maintain electrolyte balance.

4. Give appropriate antidote according to the drug situation. Patients with acute poisoning can take measures such as gastric lavage, catharsis, activated carbon adsorption to eliminate gastrointestinal residual drugs, and use hemodialysis, peritoneal dialysis, hemoperfusion, plasma exchange and other methods to quickly remove drugs from the body; the use of antidotes: including non-specific such as glutathione, N-acetylcysteine, sodium thiosulfate, steroid hormones, UDCA, S-adenosylmethionine, polyene phosphatidylcholine, etc. and specific chelating agents such as dimercaptopropanol, penicillamine, succinic acid, thioethylamine, sodium calcium edetate, etc.

5. Patients with obvious cholestasis can try prednisone treatment. Anti-inflammatory and liver-protecting drugs can be appropriately selected according to the patient's clinical condition, including glycyrrhizic acid preparations and silymarin for anti-inflammatory and liver-protection, siopromine, reduced glutathione, and N-acetylcysteine ​​for anti-free radical damage, polyenylphosphatidylcholine for protecting liver cell membranes, and promoting liver cell metabolism: adenosylmethionine, glucuronide, complex coenzymes, potassium magnesium aspartate, hepatocyte growth factor for promoting liver cell repair and regeneration, adenosylmethionine, potassium magnesium aspartate, ursodeoxycholic acid, etc. for promoting bilirubin and bile acid metabolism.

6. Complicated with fulminant hepatic failure should be treated according to the principles of fulminant hepatitis. Artificial liver or artificial kidney can be used to clear drugs, and special antidotes can be applied. Paracetamol-induced liver necrosis can be detoxified with N-acetylcysteine. For patients with severe and rapidly progressing disease, liver transplantation may be the only effective treatment.
