After activities, you may feel palpitations, shortness of breath, difficulty breathing, and decreased work endurance, and there are varying degrees of cyanosis and other hypoxia symptoms. Physical examination may reveal obvious signs of emphysema, such as barrel chest, clear lung percussion sounds, generally weakened breath sounds on auscultation, and dry and wet rales can often be heard. Although the right ventricle is enlarged, the heart dullness is often difficult to detect due to the presence of emphysema, and the heart sounds are distant. The second sound in the pulmonary valve area is hyperactive (this sign may not be obvious due to the presence of emphysema), indicating the presence of pulmonary hypertension. 2. The systolic beat of the heart can be seen under the xiphoid process, and the systolic murmur can be heard in the tricuspid valve area. The heart sounds under the xiphoid process are stronger than those at the apex, which often indicates right ventricular hypertrophy and enlargement. In some cases, severe emphysema causes increased intrathoracic pressure, which obstructs the vena cava return and may cause distension of the jugular vein. Because the diaphragm descends, the lower edge of the liver can be touched under the ribs, which resembles the signs of right heart failure. However, at this time, the venous pressure is not significantly increased, the liver is not congested, the anterior-posterior diameter is not enlarged, and there is no tenderness, so it can be differentiated. 3. Pulmonary and cardiac decompensation stage? The main clinical manifestations of this stage are respiratory failure and heart failure. Most patients suffer from respiratory failure first, followed by heart failure. A small number of patients may not have heart failure or suffer from heart failure first. 4.
