2. Perianal fungal infection: Especially in hot and humid seasons, elderly people with obesity and excessive sweating are prone to skin fungal infections and tinea cruris, which often cause perianal itching due to the expansion of skin lesions to the area near the anus. The main symptoms are the appearance of annular, semi-circular or map-like erythema with clear boundaries on the inner thigh or buttocks, with a layer of white scale on the surface. 3. Perianal viral infection: Anal condyloma acuminatum or molluscum contagiosum may also cause anal itching (itching is a common symptom of many skin diseases. If there is only skin itching without primary skin damage, it is called pruritus.). The former is mainly manifested by the appearance of cauliflower-like or cockscomb-like wart-like growths around the anus, which are damp, dirty and have a foul odor; the latter has skin lesions that appear as round or hemispherical wart-like growths with a "navel"-like depression in the center. In addition to the perianal area and scrotum, it can also affect the skin of the entire body.

4. Anal parasitic infection: usually seen in children, but adults can also be infected and itching is most common at night. When the anus relaxes at night, pinworms often crawl out of the rectum, lay eggs and mate around the anus, which irritates the skin around the anus and causes anal itching, which is characterized by nocturnal itching. When suffering from scabies, if the skin lesions on the vulva spread to the area near the anus, it may also cause anal itching. In addition, the irritation of leucorrhea in elderly people with Trichomonas vaginitis can also cause anal itching.

5. Perianal eczema: The main manifestations are papules, blisters, and even skin lesions such as erosion and exudate around the anus. Severe itching and repeated scratching often cause the skin around the anus to thicken and lichenify, and the appearance mostly becomes grayish white.
