Diagnosis/differentiation of gastric reflux symptoms

The upward flow of stomach qi is also called vomiting. If there is sound and substance, it is called "vomiting", if there is substance but no sound, it is called "spitting", and if there is sound but no substance, it is called "dry retching". In clinical practice, vomiting and vomiting often occur at the same time, so they are collectively called "vomiting". It can occur in both men and women, young and old, and is a common and frequently occurring clinical disease.

Diagnostic points:

① Typical clinical symptoms: frequent hiccups, continuous or intermittent, and inability to control;

② History of eating raw or cold food, eating too much spicy food, or having emotional stimulation.

In Lingshu Kouwen, it is stated that "when food enters the stomach, the stomach qi goes up to the lungs. There will be old cold air and new food qi, both of which return to the stomach. The new and the old are confused, the true and the evil attack each other, the qi go against each other, and come out of the stomach again, so it causes hiccups." This explains that hiccups are caused by the stomach qi going against the diaphragm. Based on this, later doctors concluded that hiccups can be divided into deficiency or excess, cold or heat. It is caused by improper diet, emotional distress, liver qi rebellion, or depletion of vital energy. This shows that hiccups are located in the diaphragm and stomach, but are closely related to the functions of other internal organs.

Symptoms of gastric reflux

The pathogenesis of stomach qi reflux is clear, so the clinical diagnosis and treatment should combine the liver, spleen and stomach, to soothe the liver and strengthen the spleen to treat the root cause, and to unblock and descend the stomach qi to treat the symptoms. It is possible to disperse without damaging the vital energy, replenish without hindering the flow of qi, and reduce without damaging the stomach qi. Acute nausea is usually caused by excessive evil, which is easier to diagnose and treat. Chronic nausea is often caused by deficiency of the body's vital energy, and requires careful observation and identification.

The medical consultation should be patient, comprehensive, detailed and objective. In addition to understanding the frequency of symptom attacks, the length of the disease course, whether the attacks are intermittent and the causes of recurrence, it is also important to understand the medical history of other relevant organs based on the specific circumstances, especially to clarify whether there is a history of chronic diseases. Pay attention to the presence of cervical and chest diseases: cervical or mediastinal tumors, esophageal diverticulum, myocardial infarction, pericarditis, pneumonia, pleurisy, diaphragmatic hernia or foreign bodies.
