Spina bifida is a group of disorders that are abnormalities of the developing spinal canal in children and are defined as incomplete or absent fusion of the mesenchymal, bony, and neural structures in the midline of the trunk.

Surgical method:

1. The surgical method for diastematomyelia is to remove the foreign body and release the traction of the foreign body on the spinal cord. The current popular practice is still to separate the adhered and stretched spinal cord from the bulging dura mater sac, place it in the spinal canal, completely free the spinal cord, release the traction, trim the excess dura mater, and expand and suture the dura mater sac with dura mater or a patch. The following points should also be considered during treatment: the extent and plane of the defect, whether there is severe hydrocephalus, whether there is infection, and whether there are other organ malformations.

2. Cystic spina bifida almost always requires surgical treatment. If the cyst wall is extremely thin or has ruptured, emergency or early surgery is required. In other cases, it is better to have surgery within 1 to 3 months after birth to prevent the cyst wall from rupturing and worsening of the disease. If the cyst wall is thick, in order to reduce the surgical mortality rate, the child can also undergo surgery when he is older (after 1 and a half years old). The purpose of the operation is to remove the bulging cyst wall, loosen the adhesions between the spinal cord and nerve roots, return the bulging nerve tissue to the spinal canal, repair the soft tissue defects, and avoid persistent traction of the nerve tissue and aggravation of symptoms. When the spinal opening cannot be sutured directly, the dorsal fascia should be turned over for repair. The bandage should be tight, and the patient should lie prone or side-lying for 2 to 3 days after the operation and after the removal of the sutures to prevent urine and feces from soaking and contaminating the incision.

3. For occult spina bifida with long-term urinary disorders or nocturnal enuresis or persistent worsening neurological symptoms, appropriate surgical treatment should be given after careful examination. The goal of surgery is to remove the fibers and fatty tissue that are compressing the nerve roots.

4. For patients with complete paralysis of both lower limbs and incontinence at birth, or with myelomeningocele accompanied by obvious hydrocephalus, it is usually difficult to return to normal after surgery. It may even aggravate symptoms or cause other complications.
