Clinical manifestations Fungal infections (35%): Tinea capitis is a disease caused by fungal infection of the scalp and hair. If you suffer from tinea capitis, many gray scales or large yellow scabs will appear on the scalp. It can also cause hair to break or fall off. In severe cases, the hair will be uneven or even almost gone, which is what people call "bald head". Contact infection (25%): Tinea capitis is mainly transmitted through contact with the hair of patients with tinea capitis or sick animals. It is extremely rare to be infected from nature. Normal people often have close contact with patients with tinea capitis, especially children playing together and head to head contact, which can easily be infected. People can also become infected after contact with sick animals. This is all direct transmission. If you use hats, headscarves, pillows, combs or hair clippers, scissors, etc. used by patients with tinea capitis, you can also be infected. This is called indirect transmission. It can be seen that tinea capitis is very contagious. In the past, tinea capitis was very prevalent in my country and patients with tinea capitis could often be seen in hospitals. Resistance (25%): However, fungal infection does not necessarily cause tinea capitis. This is closely related to the body's resistance to fungi. Most adults have stronger resistance to fungi, while children have weaker resistance, so tinea capitis is more common in children. Pathogenesis A suitable environment for the host's skin is very important for the development of dermatophytosis. In addition to trauma, increased moisture soaking the skin is also suitable for the growth of dermatophytes. Non-breathable clothing can increase the local temperature and humidity of the skin and interfere with the barrier function of the stratum corneum. After dermatophytes invade the skin, they go through several stages of development, including an incubation period, enlargement during the anti-infection process, and a degeneration period. Diagnostic Methods The diagnosis can be confirmed based on clinical manifestations, morphology and location of skin lesions and microscopic examination, but it should be differentiated from neurodermatitis and chronic eczema: 1. Neurodermatitis: There is obvious lichenification, no blisters, and fungal microscopy is negative. 2. Chronic eczema: There is no ridge-like raised edge, the boundary is unclear, and the fungal test is negative. (1) Psoriasis: Scalp psoriasis lesions are obvious erythema covered with thick silvery white scales. The hair is in bundles but there is no broken hair, sterile sheaths, and fungal examination is negative. (2) Seborrheic dermatitis: The scalp has diffuse scaly patches with unclear borders or covered with greasy crusts, accompanied by hair loss, but no broken hair or fungal sheaths, and fungal examination is negative. In addition, it is necessary to differentiate it from head eczema, scalp pityriasis, etc., and perform fungal examination when necessary. Pustulariasis should also be differentiated from purulent skin abscesses such as perforating folliculitis on the head and perifolliculitis. The latter has no honeycomb-like follicular pores, obvious pain, and is often accompanied by fever. Fungal microscopy can be used for differentiation when necessary. prevention Although good hygiene habits such as washing your hair regularly can reduce dandruff, they cannot actually kill the dandruff bacteria. Medicinal bath therapy can directly apply drugs to the skin, orifices, acupoints, etc., and can quickly reach the diseased area to achieve good therapeutic effects.
