The scabies has healed but the nodules are still itchy

Generally speaking, some itching symptoms will still appear after scabies is cured. Why is this? Many friends think that scabies is caused by other diseases when the nodules are still itchy even though the scabies has already healed. Actually, it is not. This situation is because scabies has recurred and needs to be treated again. Generally speaking, patients can be treated with medication, which can achieve good results.

Scabies is a contact infectious skin disease caused by scabies mites in the epidermis of human skin. It can spread among families and contacts. The clinical manifestations are characterized by papules, blisters and tunnels on the tender skin, pruritic nodules on the scrotum, and aggravated itching at night.

Scabies mites often parasitize on areas where the skin is thin and soft, such as between the fingers and their sides, the flexion surface of the wrist, the elbows, armpits, around the navel, waist, lower abdomen, genitals, groin and inner side of the upper thigh. The head and face are not affected, except in children. The skin lesions are pinpoint-sized papulovesicles and herpes. Tunnels dug by scabies can often be found between fingers, and the female scabies can be picked out with a needle tip at the entrance of the tunnel. This is a symptom specific to scabies. Often accompanied by severe itching at night. If the skin lesions do not heal for a long time, secondary changes often occur, such as scratches, blood crusts, punctate pigmentation, eczematous changes and pustules. Some patients may develop light or reddish-brown, mung bean to soybean-sized inflammatory nodules on the scrotum, penis, etc., which are extremely itchy and are called scabies nodules. Another rare type is Norwegian scabies, a severe form of scabies that occurs mostly in people who are weak or immunocompromised. The rash is widespread and has a distinctive odor.

treat

1. General treatment

The main treatment is external scabies-killing preparations. All patients who occur in groups or are family members should be treated at the same time. The treatment procedure is as follows: Before applying the medicine, it is best to take a bath with hot water and soap. When applying the medicine, the medicine should be applied all over the body from below the neck. The areas where the rash is concentrated should be applied repeatedly and rubbed with pressure. Take a bath with hot water and soap at the end of the treatment. Change clothes and bedding promptly, and boil them in water to disinfect them or wash them and dry them in the sun.

2. Medication

(1) Commonly used anti-scabies topical drugs: ① 10% sulfur (5% sulfur for children), 3% salicylic acid ointment. ②1% γ-666 emulsion or ointment, pay attention to neurotoxicity. ③10%~25% benzyl benzoate lotion or emulsion. ④For external use, use silanol cream. ⑤ Apply 40% sodium thiosulfate solution and 4% dilute hydrochloric acid solution twice first, then apply the latter twice after it dries. Take once in the morning and evening every day for 3 to 4 days. ⑥ Apply 10% crotamiton emulsion or ointment once in the morning and evening every day for 3 consecutive days.

After the above-mentioned topical medication treatment, the patient should be observed for 2 weeks. If no new skin lesions appear, the patient can be considered cured. Because scabies eggs will develop into adults after 7-10 days. If there is no new rash after recovery but the itching continues, apply compound calamine lotion externally.

(2) Treatment of scabies nodules: ① Apply tar gel every night for 2 to 3 weeks. ②Intralesional injection of glucocorticoids (triamcinolone acetonide). ③Apply triamcinolone acetonide and neomycin ointment topically. ④ Cryotherapy.

(3) For patients with severe itching caused by oral medication, antihistamines may be used as appropriate, and antibiotics may be added for patients with secondary infection.
