Types of acute common bacillary dysentery

Acute ordinary bacillary dysentery is a relatively common form of granuloma. It is also divided into acute bacillary dysentery and chronic bacillary dysentery. Acute bacillary dysentery has a rapid onset and often causes patients to experience symptoms such as fever, abdominal pain, diarrhea, and severe cases may cause bloody stools. If the classification is more detailed, it can also be divided into mild, ordinary and severe types. Let’s take a look at this aspect below.

Types of acute common bacillary dysentery

Acute bacillary dysentery is equivalent to damp-heat dysentery in traditional Chinese medicine. Its main symptoms include systemic poisoning symptoms and intestinal symptoms. According to its severity, it can be divided into four types: mild, ordinary, severe and poisoning.

Lightweight

⑴ Mild: There are usually no systemic poisoning symptoms, normal or slightly high body temperature, no obvious abdominal pain, diarrhea 3 to 5 times a day, yellow paste or watery stool with a small amount of mucus, no obvious tenesmus, and vomiting may occur. The course of the disease is about 3 to 6 days. It is easy to misdiagnose as enteritis or colitis, and if not treated promptly or thoroughly, it can easily turn into a chronic disease.

Common

⑵Common type: Acute onset, moderate toxemia, chills, fever, body temperature can be as high as 39 ℃. At the same time as the fever or a few hours later, digestive tract symptoms such as abdominal pain and diarrhea appear. The abdominal pain is mostly paroxysmal and located around the umbilicus or in the left lower abdomen. Diarrhea occurs 10 to 20 times a day or more. The stool is initially watery or yellow and pasty, and then the stool gradually decreases. After the intestinal contents are emptied, it turns into mucus, pus and blood in the stool, with a small amount each time, accompanied by obvious tenesmus, tenderness in the left lower abdomen, and hyperactive bowel sounds. In severe cases, dehydration and metabolic acidosis may occur. The course of the disease is 1 to 2 weeks.

Heavy

⑶ Severe cases: There are usually severe symptoms of poisoning, with sudden onset, high fever, vomiting, frequent bowel movements leading to incontinence, stools with pus, blood and mucus, severe abdominal pain, a significant feeling of tenesmus, obvious water loss, tenderness in the whole abdomen, especially in the left lower abdomen, followed by extreme exhaustion, cold and wet limbs, confusion, delirium or convulsions, and a drop in blood pressure to shock.

Poisoning

⑷ Poisoning type: mostly occurs in children aged 2 to 7 years old with good physical constitution, which is equivalent to "epidemic dysentery" in traditional Chinese medicine. The onset is acute. Before abdominal pain and diarrhea appear, the patient may have symptoms such as high fever, mental depression, pale complexion, cold limbs, weak and shallow breathing, repeated convulsions, confusion, pale skin, and prolonged nail bed capillary filling time, which may eventually lead to respiratory and circulatory failure. This type is characterized by severe toxemia, shock and toxic encephalitis, while diarrhea and vomiting are not necessarily severe and appear later. The frequency of bowel movements may not be frequent and the nature of the bowel movements may not be pus-like or bloody. The vomit may be brown. This type can be divided into:

① Shock type: mainly manifested by peripheral circulatory failure, characterized by skin rashes, bluish lips and nails, a significant drop in blood pressure or undetectable blood pressure, and varying degrees of consciousness disturbance;

② Cerebral edema type: mainly manifested by increased intracranial pressure or brain herniation, with irritability and drowsiness, normal to slightly elevated blood pressure, frequent vomiting, and increased breathing in the early stage; in the late stage, there may be coma, frequent convulsions, significantly increased blood pressure (which eventually drops), large or small or uneven pupils, slow or absent reaction to light, increased muscle tone, inward rotation of limbs, uneven breathing, irregular rhythm, sigh-like breathing, and finally slowing down or stopping.

③Mixed type: It has symptoms of both types above and is the most dangerous type.
