One of the reasons why normal gastric peristalsis can push digested food into the duodenum instead of backflowing into the esophagus is that the stomach pushes It goes forward rather than backward, and more importantly, the sphincter at the lower esophageal end is there to strictly control the flow of food - it only allows food to enter the stomach and not flow back. Once the sphincter pressure drops and becomes poorly closed due to various diseases such as ulcers, gallbladder disease, surgery, and medication, or the stomach's propulsion direction is reversed - pushing backwards, and the esophagus cannot push the refluxed gastric acid and food back into the stomach in time, problems will occur. The lining of the esophagus (medically known as the mucosa) is a stratified squamous epithelium that cannot withstand acid erosion. Once the esophageal mucosa is eroded and digested by acid, heartburn symptoms will appear through nerve reflex. Therefore, heartburn means that there is reflux of gastric fluid into the esophagus. In mild cases, it affects the patient's quality of life. In severe cases, it can cause esophagitis, esophageal ulcers and even bleeding, which are all risk factors for esophageal cancer.
