The main clinical manifestations of peritonitis are peritoneal irritation symptoms in the early stages, such as abdominal pain, abdominal muscle tension, and rebound pain. In the later stage, due to infection and toxin absorption, the main symptoms are systemic infection and poisoning. 1. Abdominal pain This is the main symptom of peritonitis. But they are usually severe, unbearable, and persistent. Taking a deep breath, coughing, or turning the body can aggravate the pain. Therefore, the patient cannot change his body position. The pain usually starts from the primary lesion, and after the inflammation spreads, it spreads to the entire abdomen, but it is still most obvious at the primary lesion site. 2. Nausea and vomiting This is a common symptom that occurs early. Initially, the irritation of the peritoneum causes reflex nausea and vomiting, and the vomitus is the contents of the stomach. When paralytic ileus occurs in the later stage, the vomitus turns yellow-green or even brown-brown fecal-like intestinal contents. 3. Fever In cases of sudden onset peritonitis, the temperature may be normal at first and then gradually rise. For elderly and frail patients, the body temperature does not necessarily rise as the condition worsens. The pulse rate usually increases as body temperature rises. 4. Infection and poisoning When peritonitis enters a severe stage, symptoms of systemic poisoning such as high fever, dry mouth, rapid pulse, and shallow breathing often appear. In the later stage, due to the absorption of a large amount of toxins, the patient will have a dull expression, haggard face, sunken eye sockets, cyanotic lips, cold limbs, yellow and cracked tongue, dry skin, rapid breathing, weak pulse, sharp rise or fall in body temperature, low blood pressure, shock, and acidosis. 5. Abdominal signs It manifests as weakened or disappeared abdominal breathing, accompanied by obvious abdominal distension. Tenderness and rebound tenderness are the main signs of peritonitis. They are always present, usually throughout the abdomen and are most prominent at the site of the primary lesion. |
