Intermuscular hemangioma

When people find themselves experiencing unexplained pain in a certain part of their body, they must pay attention to intermuscular hemangioma, especially among adolescents, who are more likely to suffer from this disease, which has a lot to do with congenital factors. The symptoms of hemangioma are more obvious, sometimes with the occurrence of local lumps and obvious pain. The limbs may also have functional disorders, such as affected limb movement and inability to lift them.

Muscle cavernous hemangioma is a congenital developmental abnormality of blood vessels, characterized by the extension and expansion of vascular tissue to form cavernous sinus-like cavities. Cavernous hemangiomas of the limb muscles are rare in clinical practice. Foreign literature reports that cavernous hemangiomas of the limb muscles only account for 0.18% of hemangiomas. This disease is more common in adolescents, and there is little gender difference.

Symptoms

1. Local mass: local mass or swelling of the affected limb can be seen. The mass is soft with unclear boundaries and can be reduced by squeezing and elevating the affected limb. In some cases, the mass is not obvious, and only careful examination can reveal limb swelling and increased circumference.

2. Pain: There may be local pain and discomfort. The degree of pain varies and worsens with activity. Severe pain can affect limb function, especially near joints. The pain is related to venous thrombosis or muscle contraction obstructing blood flow, thrombophlebitis, vascular embolism, traumatic inflammation and other causes. Some tumors are closely related to nerves, compressing the nerves and causing neuralgia. In this group, there was a case in which an elbow tumor wrapped around and compressed the radial nerve, causing numbness and pain in the lateral forearm.

3. Limb dysfunction: Limb muscle cavernous hemangioma also commonly causes limb dysfunction. The possible reasons are: (1) When the muscles of the limbs are moved and contracted, the tumor may be compressed or its blood flow may be blocked, causing severe pain. In order to relieve the pain, a special posture and gait are adopted; (2) The hemangioma invades multiple muscles and affects muscle contraction; (3) The hemangioma invades the joints and causes joint movement disorders. In one case of elbow hemangioma, the lesion was found to have invaded the joint capsule and synovium of the anterior elbow, and the joint space was also partially filled with tumor tissue; (4) The hemangioma is extensively calcified, causing muscle contracture and joint deformity and ankylosis.

4. X-ray examination: Soft tissue masses or reticular increased density shadows can be seen on the X-ray film, and phleboliths can be seen in about half of the cases. Phleboliths are caused by phlebitis and calcification of clots in the cavernous cavity, which is a characteristic feature of the diagnosis of cavernous hemangioma. In a few cases, bone destruction, localized periosteal hyperplasia, and extensive muscle ossification may be seen on X-rays.
