CO2 laser (usually used power 25W, focused); and Nd:YAG laser (using power 30-60W) are commonly used for surgical treatment. The length is disinfected with 5% iodine tincture, and then deiodinated with 75% ethanol for standby use. The surgical site is strictly disinfected according to the requirements of surgical operations, and the disinfection area is the same as that of surgery, and the dressing is applied. The CO2 laser blade is protected by a sterile coat (the coat is homemade). Squamous cell carcinoma is cut parallel to the CO2 laser knife, accompanied by ulcer changes, and the laser knife cuts at a nearly 90-degree angle to the skin. The treatment surface should be uniform and flat, and can be treated 0.5-1cm beyond the ulcer surface; the depth depends on whether the tumor has invaded or not, and the suspected one reaches the subcutaneous tissue.

Nd:YAG laser treatment is used for patients with larger areas and special locations. The surgeon holds sterile gauze in his left hand and a laser knife in his right hand (quartz laser knife can be equipped with a knife handle (self-made) according to the cutting method of the lesion). Insert and cut parallel to the base of the tumor to reach the normal tissue. Because there is no bridge between tumor cells, the normal physiological tissue function is lost. It is extremely important to judge whether the normal tissue is reached during the operation. For small-area skin tumors, Nd:YAG laser knife is used to directly insert into the base, output high-energy laser, and insert into the tumor at multiple points to completely and thoroughly destroy the tumor cells. In terms of tissue structure, tumor cells are quite different from normal cells, and the tumor can be killed at a temperature of 40°. After insertion treatment, there is no need to peel off the laser-cut tissue, allowing it to repair and fall off naturally, which also plays a role in protecting the scab.

There is a lot of bleeding during surgery for giant squamous cell carcinoma. The author treated a patient with giant squamous cell carcinoma of the head (cancer lesion 8×10cm) by inserting high-energy laser into the tumor and performing parallel basal cutting. The patient lost about 150-200mL of blood during the operation. After the laser treatment, the bleeding was quickly controlled by applying pressure with the hand for a few moments. The surgical operation should be steady and fast.

For patients with regional lymph node metastasis, use CO2 laser or Nd:YAG laser to cut the skin according to the surgical requirements. After clamping the inner edge of the incision with skin clamps and gently pulling, use a laser knife to peel off the subcutaneous tissue, including the fat tissue around the subcutaneous lymph nodes, without leaving any. When cutting deep, use skin clamps to clamp the lymph node and gently lift it upward. Note that the lymphatic tissue is fragile and should not be clamped too hard to cause lymph rupture and iatrogenic transmission. It is best to clamp the fibrous tissue attached to the separated lymph node. After the cutting is completed, the wound is sutured according to the anatomy. If there is suspected contamination during the operation, a rubber drainage strip is placed in the incision. Observe the secretions from the sutured incision 24 hours after the operation. If there is a lot of secretion, the drainage should be appropriately extended; if there is little secretion, it should be removed 24 hours after the operation to keep the wound sterile until it is completely healed.
