Why is liver cancer not easy to detect?

In the 1960s, among the liver cancer patients found in my country, less than 1% were stage I patients without symptoms or signs. In recent years, with the increase of people's health awareness and the improvement of diagnostic methods, the proportion of stage I liver cancer patients has increased, but it is still less than 1/5 of all liver cancer cases. It can be seen that liver cancer has an insidious onset and is difficult to diagnose. Once there are obvious symptoms and the diagnosis is clear, the disease has usually entered the middle or late stage, thus losing the opportunity for early treatment. The reason why liver cancer is not easy to detect early is:

1. Diversity and non-specificity of early symptoms of liver cancer: These symptoms often do not attract the attention of patients. Even if they do, most of them do not consider the possibility of liver cancer because these symptoms are mostly non-specific. The early symptoms of liver cancer are often mild, diverse but non-specific. Such as upper abdominal distension and pain, fever, fatigue, diarrhea, loss of appetite, etc. A small number of patients show symptoms of metastatic lesions, such as cough, hemoptysis, chest pain when palpating lung and chest metastasis, and back pain when invading the pancreas.

2. Hidden onset: There are basically no conscious symptoms in the early stages of liver cancer, and no positive findings are found in routine physical examinations. At this time, liver cancer cannot be discovered or diagnosed unless the patient undergoes relevant imaging examinations regularly.

3. Patients have a weak health awareness: If they can have regular physical examinations, they can detect, diagnose and treat the disease early. But in fact, not many people can have regular physical examinations. Therefore, people in high-incidence areas of liver cancer, natural populations, especially high-risk groups, such as patients over 35 years old with chronic liver diseases such as chronic hepatitis and cirrhosis, should increase their awareness of health care and physical examinations.

4. Medical reasons: In some remote areas, poor medical conditions and backward diagnostic methods are also one of the reasons for the delay in timely diagnosis. Due to the diversity of liver cancer manifestations, some scholars have divided it into nine types according to clinical manifestations: common, sclerosis, latent, high fever, metastasis, hepatitis, acute abdomen, cholestasis and others. This shows the complexity and lack of specificity of its clinical manifestations. Therefore, doctors must be vigilant to avoid misdiagnosis.

5. Masked by background diseases: The fear of cancer in some patients can delay diagnosis. Liver cancer patients often have hepatitis and cirrhosis, so some of the patient's clinical manifestations such as jaundice, ascites, fatigue, abdominal pain, etc. are often considered to be manifestations of previous diseases.
