(1) Pain in the liver: It is the most common and main clinical symptom of primary liver cancer. The pain is usually persistent dull pain, bloating or stabbing pain, which is more obvious at night or after fatigue. Pain in the liver is caused by the rapid growth of the tumor, which increases the tension of the liver capsule or ruptures the cancer nodules under the capsule or ruptures and bleeds the liver cancer nodules. The location of pain in the liver is closely related to the location of the lesion. If the lesion is located in the right lobe of the liver, it may manifest as right hypochondrium pain; if it is located in the left lobe of the liver, it may manifest as epigastric pain; if it is located behind the diaphragm, the pain may radiate to the shoulders and waist and back. (2) Poor appetite, nausea and vomiting: often caused by liver damage or tumor compression of the gastrointestinal tract. Poor appetite is the most common symptom, and the more severe the condition, the more obvious the symptoms.

(3) Abdominal distension: Abdominal distension is caused by large tumors, ascites, and liver dysfunction. It is more obvious in the upper abdomen, especially after meals and in the afternoon. Patients with primary liver cancer often reduce their food intake to try to alleviate the symptoms, which is often mistaken for indigestion and is not taken seriously, delaying diagnosis and treatment.

(4) Fatigue and weight loss: Due to excessive metabolic consumption of malignant tumors and lack of food intake, primary liver cancer may not be obvious in the early stages. As the disease progresses, the body weight gradually decreases. In the late stages of primary liver cancer, patients are often extremely emaciated and anemic, and suffer from cachexia. A small number of primary liver cancer patients with slower disease progression may also experience temporary weight gain after rest and supportive treatment.

(5) Diarrhea: It is mainly caused by the decrease of digestion and absorption capacity due to liver damage to varying degrees. It can also be caused by the metastasis of liver cancer cells to form portal vein cancer thrombus. Although this symptom is not very common, it can sometimes be the first symptom of primary liver cancer. It is often mistaken for gastrointestinal infection and misdiagnosed as diarrhea. It may not be accompanied by abdominal pain. Generally, diarrhea occurs after eating. The stool is mostly undigested food residues, often without pus and blood. Anti-inflammatory drugs cannot control the disease. In severe cases, the patient may have more than ten bowel movements a day, which can rapidly worsen the disease.

(6) Fever: Fever caused by tumor tissue necrosis, increased metabolic products, and tumor compression of the bile duct combined with cholangitis is called cancer fever in non-infected patients. It is usually not accompanied by chills. Unexplained low-grade fever is a common symptom of primary liver cancer, and the body temperature is generally between 37.5℃ and 38℃. However, inflammatory diffuse liver cancer often has a high fever with a temperature of over 39℃, which can be easily misdiagnosed as liver abscess. Antibiotic treatment is often ineffective, but indomethacin (indomethacin) can reduce fever.
