The early symptoms of liver tumors in children are often irregular localized liver enlargement, with the tumor located in the right abdomen or right upper abdomen. The medical history often records that the tumor grows rapidly, some of which can reach below the navel or beyond the midline, with a smooth surface, clear edges, medium hardness, slight movement to the left and right, and no tenderness.

In the early stage of liver tumor in children, except for mild anemia, the general condition is generally good. In the late stage, jaundice, ascites, fever, anemia, weight loss, engorgement of veins in the abdominal wall, and breathing difficulties due to the huge mass in the abdomen may occur. About 20% of hepatoblastoma cases have osteoporosis, and severe cases can lead to multiple fractures.

Many patients present with anemia and thrombocytosis when they see a doctor, especially in children with hepatoblastoma. The liver function of children with hepatoblastoma is often normal, but in cases of hepatocellular carcinoma, serum bilirubin, alkaline phosphatase, and transaminases may be elevated due to complications of hepatitis or cirrhosis. Alpha-fetoprotein is elevated in 60% to 90% of hepatocellular carcinoma cases and more than 90% of hepatoblastoma cases. Children with hepatoblastoma have increased urinary excretion of cystathionine.

Other common primary liver tumors include cavernous hemangiomas and hemangioendothelioma. The former can compress liver tissue and cause liver cell degeneration due to tumor growth. Sometimes there is an arteriovenous shunt in the tumor, which can cause heart failure in children or death due to tumor rupture and bleeding. Small hemangiomas grow slowly and may have no clinical symptoms.

Hemangioendothelioma is a malignant tumor with blood sinus formation in the liver. Clinically, the symptoms of liver tumors in children also include pain, high fever and jaundice. The course of the disease is slow, but the prognosis is poor.

Hepatic hamartoma, teratoma, and single or multiple hepatic cysts are extremely rare.
