The esophageal ligature is fixed on the bougie below the site to be anastomosed, the upper esophagus is cut off, and the bougie is pulled continuously and evenly to flip and pull the esophagus from top to bottom.

The free gastrectomy is routinely performed to remove part of the stomach, cut the greater curvature into a tube, and lift it up through the esophageal bed into the neck and anastomose it with the esophagus. The disadvantage of this type of surgery for treating cardiac cancer is that the scope of gastrectomy is limited, which can lead to incomplete gastric resection margins and residual cancer. When there is inflammation in the mediastinum, such as lymphadenopathy and adhesion, it will be difficult to flip and remove the tumor, and there may be a situation where the tumor cannot be removed or the tracheobronchial membrane is torn. Once the latter situation occurs, open-chest repair should be performed immediately. If it is estimated in advance that there may be difficulty in removal, it is recommended to directly use open-chest resection for treatment.
