Surgical treatment of pancreatic cancer

Surgical resection is the most effective way to treat pancreatic cancer, which can effectively prolong the patient's survival time and improve the quality of life. There are many different forms of surgical treatment for pancreatic cancer. The commonly used surgical methods are as follows:

1. Radical resection of pancreatic cancer

Radical surgery for pancreatic cancer can effectively improve the survival of patients. Radical surgery for pancreatic cancer is divided into pancreaticoduodenectomy, pancreatic body and tail resection and extended pancreatic cancer radical resection. Pancreaticoduodenectomy (Wipple surgery), the resection range includes the head of the pancreas, distal stomach, duodenum, upper jejunum, gallbladder and common bile duct. After resection, digestive tract reconstruction and removal of related lymph nodes are required; pancreatic body and tail resection: pancreatic body and tail cancer with a diameter of less than 2cm, no pancreatic cell membrane invasion, no peripancreatic lymph node metastasis, and no distant metastasis; extended pancreatic cancer radical resection: including regional pancreaticoduodenectomy and regional total pancreatectomy.

2. Palliative surgery

It is suitable for patients who are elderly, have liver metastases, whose tumors cannot be removed, or who have obvious cardiopulmonary dysfunction and cannot tolerate major surgery. The treatment aims to remove local or partial lesions, relieve symptoms, and reduce pain. It can prolong the patient's survival to a certain extent and effectively curb the spread of cancer cells.

3. Laparoscopic surgery

Laparoscopic surgery for pancreatic cancer is suitable for patients with advanced pancreatic cancer, especially those who have developed bile duct obstruction and gastrointestinal obstruction. Laparoscopic choledochojejunostomy or gastrointestinal anastomosis is used. Laparoscopic intraperitoneal drainage has less trauma, shorter operation time, less bleeding, fewer surgical complications, lower mortality, and faster resumption of normal activities.
