Treatment principles for Hodgkin's lymphoma

The treatment principle of Hodgkin's lymphoma is comprehensive treatment with chemotherapy as the main treatment and radiotherapy combined.

Hodgkin's lymphoma is a malignant tumor that can be completely cured by chemotherapy. The chemotherapy regimen group must include MOPP and ABVD. The MOPP chemotherapy regimen takes a long time, at least 6 courses of treatment, or until the patient is completely relieved, and then 2 courses of consolidation. Three-quarters of patients can achieve a 5-year survival rate. This regimen is still effective for patients with recurrence, but this regimen has adverse reactions that affect reproductive function and cause secondary tumors. The ABVD regimen is superior to the MOPP regimen, has little effect on reproductive function, and does not cause secondary tumors. It has gradually replaced the MOPP regimen. The regimen must be strictly controlled when used, because adding maintenance treatment does not prolong survival, but only increases chemotherapy toxicity and suppresses immune function.

There are two radiotherapy methods for lymphoma. If the lesion is above the diaphragm, the cloak-style radiation is used. The irradiated parts include the lymph nodes on both sides from the mastoid end to the clavicle, axilla, hilum, mediastinum to diaphragm. Attention should be paid to protecting the humeral head, larynx and lungs from radiation. If the lesion is below the diaphragm, the inverted "Y" radiation is used, including the subdiaphragmatic lymph nodes to the para-aortic, pelvic and inguinal lymph nodes, and the spleen area is irradiated at the same time. The radiation dose is 30-40Gy, and one course of treatment is 3-4 weeks.

For pediatric patients, chemotherapy is the first choice to avoid permanent damage caused by radiotherapy that delays their development. For large tumors or residual tumors after chemotherapy, adjuvant radiotherapy should be added.
