Things to note when patients with gallbladder cancer undergo cholecystectomy

Gallbladder cancer is a malignant tumor of the digestive tract with a high incidence rate. Patients with more serious cancer usually need to undergo cholecystectomy. In order to improve the success rate of the operation and reduce the pain of the patient to the greatest extent, the aseptic operation principle should be strictly followed during the cholecystectomy operation, and the operation should be accurate to avoid damaging other tissues.

1. There are extrahepatic bile duct and cystic artery near the gallbladder, which often mutate. When the tissues become inflamed, it is easy to cause adhesion between the tissues. Doctors must be cautious during operation and carefully identify them.

2. During the operation, when clamping and cutting the cystic duct, the common bile duct and cystic artery may be damaged if you are not careful. Therefore, the doctor needs to be very familiar with the anatomical structure of the abdominal gallbladder and liver.

3. The stump of the cystic duct should not be too long to avoid expansion of the area in the future, causing bile stasis and infection; but it should not be too short to avoid stenosis of the common bile duct when ligating the common bile duct. It is generally appropriate to keep it 0.3 to 0.5 cm away from the common bile duct.

4. If the gallbladder and gallbladder bed are tightly adhered, in order to avoid damaging the liver, just gently peel off the gallbladder mucosa until the cystic duct, which can achieve better results and avoid involving other organs.

5. Sometimes the patient's cystic duct is very long, parallel to the common hepatic duct, and closely adheres downward, reaching the duodenum or the back of the pancreas to merge with each other. With such a complex path, it is easy to leave too long cystic duct when ligating the cystic duct, resulting in the formation of a blind sac after surgery, which may gradually increase in size in the future and easily cause the occurrence of stones.
