Whether breast cancer patients can have children is a more complicated issue. If the estrogen receptor (ER) and progesterone receptor (PR) test results of the patient are negative, that is, the patient does not need endocrine therapy, we recommend that she should not get pregnant within three years after the surgery and chemotherapy, because the incidence of tumor recurrence and metastasis is relatively high in the first three years. If after three years, the patient is in good physical condition and has the desire to have children, she can get pregnant.

Because this treatment requires continuous medication for five or even ten years, these drugs may have a certain impact on the fetus, so contraception should be strictly followed during the medication period. But is it absolutely impossible to get pregnant during treatment? There is still controversy. For patients with hormone receptor-positive, who need to receive endocrine therapy, we do not encourage childbearing during treatment. But assuming that the patient has this need, and the tumor is currently relatively stable and there is no sign of recurrence or metastasis, if pregnancy is considered, the patient needs to carefully weigh the pros and cons. Because if you want to get pregnant, you have to stop taking the medication and interrupt the treatment of the tumor. How big is this risk, and whether it is worth taking this risk for pregnancy, it is difficult to weigh from a doctor's perspective.

If you can have a baby and the tumor is fine, that would be a perfect result; but the worst result might be not being able to keep the baby, and the tumor recurring or metastasizing. So these patients need to carefully consider the risks. The patient's age should also be considered. For example, if the patient is 25 years old, he will be 30 years old after 5 years of observation after treatment, and it is not too late to consider having a baby afterwards; but if the patient is older and still has no children at around 35 years old, he will be 40 years old after the treatment. At this time, the chance of a woman getting pregnant will drop significantly, and the patient needs to weigh the risks and decide whether to get pregnant.

Some patients are also worried that they may stop menstruating or go into menopause after breast cancer treatment, and they are worried that they will not be able to get pregnant without menstruation. This depends on the situation. Patients who have gone into menopause will basically not get pregnant again; although the chance of pregnancy for patients who have stopped menstruating is relatively low, if the ovarian function can be restored after menopause, then there is still a chance of getting pregnant again, but it may be difficult.

It should be emphasized that menstruation is not a prerequisite for pregnancy. Some patients can ovulate quietly, and ovulate even if the endometrium does not change (it is possible to ovulate even if there is no menstruation), and they may happen to conceive. Of course, without periodic menstruation, the endometrium may not be fertile enough, and the egg may not be easy to successfully implant, which will lead to infertility or even miscarriage. This also reminds breast cancer patients from another aspect that it is possible to get pregnant accidentally during postoperative treatment. Although menstruation may not be restored at that time, there is a risk of pregnancy when ovarian function is restored, so contraceptive measures need to be taken during treatment.
